DCE-MRI联合DWI对乳腺可疑恶性病变的诊断价值

The value of DCE-MRI combined with DWI in identification of suspected malignant breast lesions

摘要: 目的 探讨乳腺动态增强磁共振成像(dynamic contrast-enhanced magnetic resonance imaging,DCE-MRI)联合扩散加权成像(diffusion weighted imaging,DWI)对乳腺可疑恶性病变的诊断价值。方法 回顾性分析了2017年1月至2018年10月间于我院行乳腺MR检查的129名患者的142个经病理证实的可疑恶性病变。收集患者临床资料及术前MRI资料。以病理结果为金标准将病灶分为良性组及恶性组,比较两组间时间-信号曲线(time to intensity curve,TIC)及表观扩散系数(apparent diffusion coefficient,ADC)的差异。用ROC曲线分析DCE-MRI、DWI及二者联合诊断的敏感性、特异性、曲线下面积(AUC)。结果 142个病灶中恶性肿瘤89个,良性肿瘤53个。DCE-MRI联合DWI诊断乳腺癌敏感性及特异性分别为88.9%、82.7%,高于DCE-MRI(77.8%、59.6%)及DWI(62.2%、69.2%)单独诊断,并且二者联合诊断时曲线下面积(AUC)最大。结论 DCE-MRI具有高敏感性,但特异性低,与DWI联合不仅提高了灵敏度,增加特异性,并最大限度地提高对乳腺可疑恶性病变诊断的准确性。

Abstract: Objective: To explore the value of DCE-MRI combined with DWI in identification of suspected malignant breast lesions. Methods: Retrospective analysis was made of 142 pathologically confirmed malignant lesions of 129 patients who underwent breast MR examination in our hospital from January 2017 to October 2018. The clinical data and preoperative MRI data were collected. The lesions were divided into benign and malignant groups according to pathological results. The differences of time to intensity curve (TIC) and apparent diffusion coefficient (ADC) between the two groups were compared. ROC curve was used to analyze the sensitivity, specificity and area under the curve (AUC) of DCE-MRI, DWI and their combination. Results: Among the 142 cases, 89 were malignant and 53 were benign. The sensitivity and specificity of DCE-MRI combined with DWI in the diagnosis of breast cancer were 88.9% and 82.7%, respectively, which were higher than those of DCE-MRI(77.8%,59.6%) and DWI (62.2%, 69.2%) alone, and the AUC was the largest when the two were combined. Conclusion: DCE-MRI has high sensitivity but low specificity. DCE-MRI combined with DWI can not only improve the sensitivity and specificity, but also maximize the accuracy of diagnosis of suspected malignant breast lesions.
