原发性肝细胞癌患者外周血中T细胞及NK细胞的水平变化和临床意义探讨

Study on the changes and clinical significance of T cells and NK cells in peripheral blood of patients with primary hepatocellular carcinoma

摘要: 目的 探讨原发性肝细胞癌患者外周血中T细胞和NK细胞的水平变化及其与癌症的关系。方法 通过对28例原发性肝细胞癌患者、29例慢性肝硬化患者、31例健康成人外周静脉血中T细胞及NK细胞的水平进行分析,研究其与肿瘤的关系。结果 原发性肝癌患者外周静脉血中,CD3+ T细胞、CD4+ T细胞、NK细胞水平明显低于慢性肝硬化患者和健康成人,差异具有统计学意义(P﹤0.05);CD8+ T细胞水平明显高于慢性肝硬化患者和健康成人,差异具统计学意义(P<0.05)。原发性肝癌患者外周静脉血中,Ⅰ期、Ⅱ期患者CD3+ T细胞、CD4+ T细胞、CD8+ T、NK细胞水平比较差异无统计学意义(P>0.05);Ⅲ期患者CD3+ T细胞、CD4+ T细胞、NK细胞水平明显低于Ⅰ期、Ⅱ期患者,CD8+ T细胞水平明显高于Ⅰ期、Ⅱ期患者,差异有统计学意义(P<0.05)。结论 原发性肝细胞癌患者外周静脉血T细胞与NK细胞水平变化反映机体免疫状态,可能与肿瘤的发生存在密切联系,对判断肝癌患者的病情及预后具有重要的临床意义。

Abstract: Objective: To investigate the changes of T cells and NK cells in peripheral blood of patients with primary hepatocellular carcinoma (HCC) and their relationship with cancer.Methods: T cells and NK cells in peripheral venous blood of 28 patients with primary hepatocellular carcinoma, 29 patients with chronic liver cirrhosis, and 31 healthy adults were analyzed to study their relationship with tumor flight.Results: In the peripheral venous blood of patients with primary liver cancer, the levels of CD3+ T cells, CD4+ T cells, and NK cells were significantly lower than those of patients with chronic liver cirrhosis and healthy adults, and the difference was statistically significant (P<0.05); the level of CD8+ T cells was significantly higher than that of the patients with chronic liver cirrhosis and healthy adult and the difference was statistically significant (P﹤0.05). In the peripheral venous blood of patients with primary liver cancer, there was no significant difference in the levels of CD3+ T cells, CD4+ T cells, CD8+ T, and NK cells in patients with stage I and II (P>0.05); CD3+ T cells, CD4+ T in patients with stage III were lower than those of stage I and II patients. The levels of cells and NK cells were significantly lower than those of stage I and II patients, and the levels of CD8+ T cells were significantly higher than those of stage I and II patients. The difference was statistically significant (P<0.05).Conclusion: The changes of peripheral venous blood T cell and NK cell levels in patients with primary hepatocellular carcinoma reflect the immune state of the body, and may be closely related to the occurrence of tumor, which is of great clinical significance in judging the condition and prognosis of patients with hepatocellular carcinoma.
