双侧子宫动脉栓塞术（BUAE）联合宫腔镜电切术治疗剖宫产瘢痕妊娠（CSP）的效果

摘要： 目的

研究分析剖宫产瘢痕妊娠患者采用双侧子宫动脉栓塞术（BUAE）联合宫腔镜电切术的治疗效果。

方法

选取2019年3月—2020年3月巨野县人民医院接收的126例剖宫产瘢痕妊娠患者作为观察对象。根据双盲随机对照原则予以分组探讨，每组63例。其中常规组采用BUAE联合传统清宫术治疗，研究组采用BUAE联合宫腔镜电切术治疗，对比两组手术疗效。

结果

研究组手术成功率高于常规组（P < 0.05）；研究组手术失血量低于常规组，且手术操作时间、月经周期恢复正常时间、血清β-HCG（人绒毛膜促性腺激素β亚单位）恢复正常时间、住院时间短于常规组（P < 0.05）；研究组术后并发症少于常规组（P < 0.05）。

结论

相比于BUAE联合传统清宫术而言，剖宫产瘢痕妊娠患者进行BUAE联合宫腔镜电切术具有疗效突出、失血量低、术后并发症少、安全性高、康复速度快等诸多优势，值得在临床中大范围普及。

Abstract: Objective

To study and analyze the therapeutic effect of BUAE （bilateral uterine artery embolization） combined with traditional cervicotomy and BUAE combined with hysteroscopic electrotomy in patients with cesarean section scar pregnancy.

Methods

126 patients with cesarean section scar pregnancy received by our hospital from March 2019 to March 2020 were taken as observation subjects.According to the double-blind randomized control principle，the study group was divided into two groups， one group was the conventional group （BUAE combined with traditional palace clearance） and the other group was the experimental group （BUAE combined with hysteroscopic electrotomy）.Each group had 63 cases.The surgical efficacy of the two groups was compared.

Results

The success rate of the study group was higher than that of the conventional group （P < 0.05）.The amount of surgical blood loss in the study group was lower than that in the conventional group， and the operation time， menstrual cycle recovery time，serum-HCG（a subunit of human chorionic gonadotropin） recovery time and hospitalization time were shorter than those in the conventional group （P < 0.05）.Postoperative complications in the study group were less than those in the conventional group （P < 0.05）.

Conclusion

Compared with traditional hysterectomy combined with BUAE， hysteroscopy combined with BUAE for cesarean section scar pregnancy patients has many advantages， such as outstanding efficacy， low blood loss， fewer postoperative complications， high safety and fast recovery， which is worth popularizing in clinic.
