目的探讨三维动脉自旋标记(3D-ASL)与动态对比增强磁共振(DCE-MRI)在鉴别脑胶质瘤复发与放射性脑坏死中的诊断价值。方法回顾性分析2016年12月至2019年12月于武汉大学人民医院40例脑胶质瘤术后放疗患者的临床及影像学资料,患者术后行放疗后影像检查出现新增异常强化灶,经二次手术或穿刺病理活检及长期影像检查随访证实,分为脑胶质瘤复发组20例,放射性脑坏死组20例。所有患者均行常规MRI、3D-ASL和DCE-MRI扫描,统计3D-ASL扫描测得的异常强化区与对侧镜像区的相对脑血流量比值(rCBF)与DCE-MRI扫描测得的容积转换常数(Ktrans),对比两组患者rCBF、Ktrans参数差异,比较常规MRI、3D-ASL、DCE-MRI及3D-ASL联合DCE-MRI鉴别诊断脑胶质瘤复发与放射性脑坏死的准确率。结果脑胶质瘤复发组患者的rCBF值(1.979±0.578)与Ktrans值(1.073±0.551)均明显高于脑坏死组(0.630±0.137,0.042±0.017),差异均有统计学意义(t=10.129,P<0.001;t=8.349,P<0.001);3D-ASL联合DCE-MRI与单纯常规MRI、3D-ASL及DCE-MRI鉴别诊断脑胶质瘤复发与放射性脑坏死的准确率分别为92.5%、67.5%、77.5%、72.5%,4种检查方法鉴别诊断的准确率差异有统计学意义(χ2=8.082,P=0.045),进一步两两比较发现,与常规MRI相比,3D-ASL联合DCE-MRI检查方法的准确率更高(χ2=7.812,P=0.005)。结论3D-ASL与DCE-MRI在鉴别脑胶质瘤复发与放射性脑坏死中具有诊断价值,与常规MRI检查相比,3D-ASL联合DCE-MRI检查可提高鉴别诊断脑胶质瘤复发与放射性脑坏死的准确率。
