    Construction of breast cancer protein risk prognosis model and identification of six key proteins

    Objective

    To optimize the prognosis assessment of breast cancer patients by developing proteomics signatures.

    Methods

    The protein expression profiles and clinical information of 879 breast cancer (BC) patient samples were downloaded from the TCPA and TCGA databases, respectively. We performed univariate Cox regression, lasso regression and multivariate Cox regression analysis to identify prognostic proteins and establish a prognostic model. Then, we further analyzed the differential expression and survival of the prognostic model protein, and analyzed the predictive performance of the model risk score. In addition, we analyzed all proteins related to prognostic model proteins.

    Results

    Based on the 224 proteins in the TCPA database, we constructed a prognostic model containing 6 proteins (CASPASE7CLIVEDD198, NFKBP65_pS536, PCADHERIN, P27, X4EBP1_pT70, EIF4G), and divided the patients into two groups according to the median of risk score. The overall survival rate was significantly different. Univariate and multivariate Cox regression analysis showed that the risk score was an independent prognostic factor for patients (P < 0.001). In addition, the nomogram and ROC are used to demonstrate the predictive performance of the predictive model. The protein interaction relationship showed that the proteins CASPASE7CLEAVEDD198, PCADHERIN, X4EBP1_pT70 and EIF4G showed obvious correlation with other proteins (P < 0.001).

    Conclusion

    Studies have shown that six key proteins may be considered as new targets for BC treatment, and the protein risk model can be used to predict the overall survival of BC patients.

    Objective

    To study the lipidomics characteristics of ankylosing spondylitis (AS) by MS for the disease lipid markers.

    Methods

    The serum samples of 61 AS patients and 28 normal persons were studied by using the technique of ultra-high performance liquid chromatography triple quadrupole mass spectrometry (UPLC/MS/MS),and the serum lipid metabolites of patients with AS and healthy people were compared.

    Results

    Serum lipid fingerprint of AS patients and the healthy people were significantly different, and the obvious differences in lipid metabolites were lysophosphatidylcholine (Lyso-GPC) and glycerin trilaurate(TG); 21 kinds of lipid metabolites with obvious difference were identified. Compared with the healthy volunteers, these 21 kinds of different lipids showed a downward trend in the serum, among which TG were the most obvious.

    Conclusion

    The 21 different lipid metabolites represented by TG(54∶7)、Lyso-GPC(18∶2) may be the lipid metabolism markers of AS, which can provide further theoretical basis for the clinical research of AS.

    The protective effects and mechanisms of astragaloside Ⅳ on memory disorder in mice induced by scopolamine

    Objective

    To study the improvement effects and antioxidant mechanisms of astragaloside Ⅳ on learning and memory disorder in mice with scopolamine.

    Methods

    The mice were randomly divided into blank control group, model group, positive control group, and the groups with high, medium and low concentration of astragaloside Ⅳ. After 14 days of continuous gavage, except for the blank group,all the other groups of mice were injected with scopolamine to prepare memory disorder models. After modeling, the dark-avoidance experiment was used to observe and record enter latency and error frequency, and the Morris water labyrinth experiment was used to observe and record each day escape latency, number of times into the target quadrant, swimming time in the target quadrant, swimming distance in the target quadrant. After the behavioral test, the levels of total superoxide dismutase (T-SOD) and malondialdehyde (MDA) in the brain tissue of each group were detected.

    Results

    Compared with blank group, the enter latency was shortened significantly in the model group (P < 0.05), error frequency was increased significantly in the model group (P < 0.05), the escaping incubation period was significantly prolonged in the model group (P < 0.05), the target quadrant number, swimming time in the target quadrant and swimming distance in the target quadrant were significantly reduced in the model group (P < 0.05), the level of MDA in the brain tissue of mice was increased significantly in the model group (P < 0.05),the activity of T-SOD was decreased significantly in the model group(P < 0.05). Compared with the model group, the enter latency of mice with high and medium concentrations of Astragaloside Ⅳ was significantly prolonged (P < 0.05), the frequency of errors of mice with high, medium and low concentrations of astragaloside Ⅳ were significantly reduced (P < 0.05), the escape latency of mice with medium concentrations of astragaloside Ⅳwas significantly shortened (P < 0.05), the number of times of entering the target quadrant, the swimming time in the target quadrant, and the swimming distance in the target quadrant were increased significantly (P < 0.05). The swimming time in the target quadrant and the swimming distance in the target quadrant of mice with high concentration of Astragaloside Ⅳ were significantly increased (P < 0.05). The levels of malondialdehyde (MDA) in the brain tissue of mice with high, medium and low concentrations of astragaloside Ⅳ were significantly detected (P < 0.05), and the activity of total superoxide dismutase (T-SOD) was significantly increased(P < 0.05).

    Conclusion

    Astragaloside Ⅳcan improve learning and memory impairment induced by scopolamine in mice, the mechanism may be related to increasing SOD level and reducing MDA level.

    Eextraction technology of diterpenoids from Leonurusjaponicas and its antimicrobial activity

    Objective

    To study the optimal extraction technology and antimicrobial activity of diterpenoids from Leonurusjaponicas.

    Methods

    After the full swelling of L. japonicas,total diterpenoids was extracted using heating reflux method. The content of total diterpenoids in L. japonicas was determined by ultraviolet spectrophotometry with andrographolide as control. Firstly, the effects of ethanol concentration, solid-liquid ratio, extraction time and extraction times on the content of diterpenoids were investigated by single factor tests. According to the single factor test results, fixing the extraction times as 3 times, the L9(33) orthogonal test of ethanol concentration, solid-liquid ratio and extraction time was conducted to optimize the extraction technology of diterpenoids from L. japonicas, and the optimized extraction technology was verified. Then the extract was further leached by ethyl acetate. Moreover, different proportions of petroleum ether and acetone were investigated using silica gel column chromatography and crude extract of diterpenoids was initially obtained. On this basis, the effect of these crude diterpenoids on antimicrobial activity of Staphylococcus aureus and Escherichia coli were studied using disk agar diffusion method.

    Results

    The optimal extraction conditions of total diterpenoids from L. japonicas were as follows∶ ethanol concentration of 90%, solid-liquid ratio of 1 g∶30 mL and extraction time of 2 h. The average content of diterpenoids from this process was 3.38%. The optimum ratio of petroleum ether to acetone for silica gel column eluent was 7∶3. In addition, the crude extract could significantly inhibit the growth of S. aureus, but had little effect on E. coli.

    Conclusion

    The optimized extraction process is stable and efficient. L. Japonicas contains active substances with antibacterial effect. This lays a foundation for further isolation and identification of active components.

    Objective

    To observe the therapeutic effect of umbilical cord blood mononuclear cells combined with marrow core decompression and bone grafting, and to provide a reference for the treatment of femoral head necrosis in early clinical stage.

    Methods

    Forty patients with early femoral head necrosis received from July 2019 to December 2019 in this department were divided into two groups according to the principle of random distribution. By observing patients' Harris score, VAS score, overall stability of femoral head X-ray, degree of femoral head collapse, and changes of MRI cystic variable area, the effect of umbilical cord blood mononuclear cells combined with core decompression and bone grafting was evaluated.

    Results

    the Harris score and VAS score in the treatment group were better than those in the control group (P < 0.05), the difference was statistically significant, the overall stability of femoral head X-ray was good, no femoral head collapse was observed, and the difference in MRI cystic change area was statistically significant compared with the control group (P < 0.05).

    Conclusion

    mononuclear cells of umbilical cord blood combined with core decompression and bone grafting are effective in the treatment of early femoral head necrosis, which provides a more effective treatment for clinical application.

    Objective

    To evaluate the efficacy of low-temperature plasma block-resection of adenoids with clamping by self-made adenoid forceps and low-tempera-ture plasma ablation,so as to guide the choice of clinical operation.

    Methods

    224 patients with adenoid hypertrophy in our department were selected. 112 cases were treated with plasma block-resection with self-made adenoid forceps as the experimental group and112 cases with plasma ablation as the control group. Operation times, bleeding volumes and complications were compared between the two groups.

    Results

    The operation time of the experimental group was significantly shorter than that of the control group, and the bleeding volumes of the experimental group are significantly less than those of the control group. No complications occurred in both groups.

    Conclusion

    Compared with plasma ablation, plasma block-resection with self-made adenoid forceps can significantly shorten the operation time and reduce bleeding volumes without more complications, which is worthy of clinical promotion.

    Objective

    To investigate the application value of laparoscopy in children with acute intussusception.

    Methods

    Retrospective analysis from November 2014 to January 2021 were surgical treatment of children with acute intussusception in 206 cases, all patients onset time > 48 hours, a jam or blood sample, the whole body with poor general condition and symptoms of dehydration video branch refused to air enema treatment failure or air enema reduction of children, Under endotracheal intubation and general anesthesia, the lesion site and status were observed under laparoscopy and the surgical method was determined.

    Results

    In this group, 71 cases were successfully reduced by pushing and pulling under laparoscopy, 28 cases were successfully reduced by laparoscopy combined with air enema, and 8 cases could not be reduced under laparoscopy, and were reduced under direct vision through right lower abdominal transverse incision. All cases underwent appendectomy.

    Conclusion

    Laparoscopic treatment of acute intussusception in children is safe and feasible with short operation time, little trauma, quick recovery. For some cases, due to long intussusception time and severe edema, laparoscopic combined with air enema is required for treatment. For refractory intussusception, surgery is required, requiring surgeons to adjust the treatment plan timely according to the condition.

    Objective

    To examine the expression levels of Gelactin-3 and VEGF in female patients before and after surgery, to analyze the therapeutic effect of laparoscopic treatment on endometrial receptivity in patients with endometriosis, and to provide more for clinical laparoscopic treatment the theoretical basis.

    Methods

    A total of 105 female patients with endometriosis who received laparoscopic treatment in our hospital from September 2018 to August 2020 were enrolled; the control group at a total of 110 cases selected in the same period due to fallopian tube infertility, benign ovarian lesions, and cervical lesions received laparoscopic treatment in our hospital, and the patients' endometrium was normal. The expression levels of Gelactin-3 and VEGF in the serum and endometrial tissue of the two groups of patients were detected to explore the improvement effect of laparoscopic treatment on patients with endometriosis.

    Results

    The levels of Gelactin-3 in the serum and endometrial tissue of patients with endometriosis before surgery were significantly lower than those in the control group, while the expression of VEGF was significantly higher than that in the control group; after surgery, endometriosis was found. The expression of Gelactin-3 was significantly increased in patients with disease, while VEGF was significantly decreased, and the difference was statistically significant (P < 0.05). There was no significant change in the levels of Gelactin-3 and VEGF in the control group before and after surgery.

    Conclusion

    Laparoscopic surgery significantly reduces the expression of VEGF and increases the expression of Galectin-3, which can improve the condition of patients with endometriosis and increase their pregnancy rate, which provides a theoretical basis for the further clinical use of laparoscopic treatment.

    Observation on short-term and long-term efficacy of thoracoscopic lobectomy and segmentectomy in the treatment of early stage non-small cell lung cancer

    Objective

    To explore the short-term and long-term efficacy of thoracoscopic lobectomy and segmentectomy in the treatment of early stage non-small cell lung cancer.

    Methods

    A total of 92 patients with early stage non-small cell lung cancer admitted to the hospital between January 2017 and January 2019 were selected as the subjects of this study. The patients were divided into observation group and control group, with 46 cases in each group, by the random coin tossing method. The control group underwent thoracoscopic lobectomy, while the observation group underwent segmentectomy. The lymphatic clearance, surgery-related indicators, postoperative complications, and quality of life score before surgery and at 1 year after surgery were compared between the two groups.

    Results

    The number of lymph node dissections, the number of mediastinal lymph node dissection stations and the number of mediastinal lymph node dissections in observation group were similar to those in the control group (P > 0.05). The surgical time of observation group was longer than that of control group, and the intraoperative blood loss, total postoperative drainage volume, postoperative hospital stay and thoracic drainage time were all less or shorter than those of control group (P < 0.05). The total incidence rates of observation group was lower than that of control group (6.52% vs 17.39%), but the difference between the two groups was not statistically significant (P > 0.05). There was no statistically significant difference in the Quality of Life Questionnaire-Core 30 (QLQ-C30) score between the two groups before surgery (P > 0.05), and the QLQ-C30 score of observation group at 1 month and 1 year after surgery was significantly higher than that of control group (P < 0.05).

    Conclusion

    Compared with lobectomy for early stage non-small cell lung cancer, thoracoscopic segmentectomy can preserve more lung tissues, and it has faster recovery, less trauma and better long-term efficacy.

    Objective

    To study the cardiac function, myocardial enzyme levels and clinical significance of 63 patients with rheumatic heart disease after heart valve replacement.

    Methods

    Sixty-three patients with heart valve replacement in our hospital from January 2019 to December 2020 were selected for the study. The CK, CK-MB, AST, α-HBD, LDH, EF and FS index changes of the patients were recorded and compared 1 day before operation, 1 day after operation, 7 days after operation and 14 days after operation.

    Results

    There were significant differences in the levels of myocardial enzymes in patients 1 day before operation, 1 day after operation, 7 days after operation and 14 days after operation (P < 0.05); The AST, α-HBD, LDH, CK and CK-MB were (53.21 ± 5.52)IU/L,(172.06 ± 20.34)IU/L,(161.17 ± 21.36)IU/L,(282.95 ± 32.14)IU/L,(16.54 ± 76) IU /L, and they were significantly lower than those of the control group(151.24 ± 16.75)IU/L,(413.58 ± 45.72)IU/L,(384.52 ± 36.27)IU/L,(657.83 ± 90.15)IU/L,(51.97 ± 6.23)IU/L,7 days after operation (116.93 ± 15.78)IU/L,(283.75 ± 30.92)IU/L,(265.63 ± 29.71)IU/L,(332.17 ± 35.74)IU/L,(30.24 ± 70) IU/L, and the difference was statistically significant (P < 0.05).With the extension of time, the AST, α-HBD, LDH, CK and CK-MB decreased gradually, and the difference was statistically significant (P < 0.05). There were significant differences in cardiac function among patients 1 day before operation, 1 day after operation, 7 days after operation and 14 days after operation (P < 0.05); the level of FS was (23.19 ± 2.15)%, significantly higher than(22.21 ± 2.09)% and (22.95 ± 2.13)%, the difference were statistically significant (P < 0.05). After the operation, the EF level of the patients increased gradually with the extension of time, and the difference was statistically significant (P < 0.05).

    Conclusion

    The changes of myocardial enzymes and cardiac function in patients with rheumatic heart disease after valve replacement can be used as the basis for judging the curative effect.

    Meta-analysis of the effect of auricular point sticking on postoperative patients with mixed hemorrhoids

    Objective

    To evaluate the effect of ear acupoint pressing on postoperative analgesia of mixed hemorrhoids.

    Methods

    Chinese and English databases such as the CNKI, VIP and Wan Fang data, CBM, Embase, Cochrane Library, PubMed and Web of Science were searched to collect randomized controlled trials (RCTs) on auricular point sticking on postoperative patients with mixed hemorrhoids published before July 2020. Two researchers conducted quality evaluation and data extraction on the included papers. Meta analysis was performed using Revman 5.3 software.

    Results

    A total of 17 RCTs involving 2282 patients were included. The results of Meta analysis showed that the total effective rate of auricular point sticking on postoperative patients with mixed hemorrhoids was [RR=1.29,95%CI(1.00,1.65)]. The pain score of the test group was lower than that of the control group at 24 hours, 48 hours and 72 hours after surgery. Four studies reported that the use of auricular point sticking to relieve postoperative pain of mixed hemorrhoids has no adverse reactions.

    Conclusion

    The auricular point sticking has a certain effect on postoperative patients with mixed hemorrhoids, and it can relive pain. However, in view of the limitations of the quality and quantity of included studies, the conclusions of this study still rely on large sample, multi-center, high quality randomized controlled trials, especially long-term intervention efficacy studies to verify.

    Objective

    To explore the influence factors of colorectal cancer, build a stable and reliable risk assessment model of colorectal cancer, and to provide rapid screening colorectal cancer high-risk groups.

    Methods

    Logistic regression model was used to analyze the influence factors of colorectal cancer, with Harvard risk index model and the age factor weighted index, build risk assessment model of colorectal cancer and choose the best the optimal prediction model according to the ROC curve area size.

    Results

    Diagnosis of intestinal polyps and bloody stool are risk factors of colorectal neoplasms, and confirmed gastritis and nervous feeling always are protective factors of colorectal cancer. AUC of ROC curve of Colorectal cancer risk assessment model was 0.789 (95% CI 0.764 to 0.813) and the prediction effect was excellent.

    Conclusion

    The influence factors of colorectal cancer is numerous, and a better risk assessment model is conducive to help grassroots doctors quickly identify high-risk groups to make the implementation of personalized intervention possible.

    Study on quality of life of patients with hypertension in rural areas of Anqing City

    Objecttive:To explore the quality of life and its influencing factors of patients with hypertension in rural areas of Anqing City, so as to provide basis for improving their quality of life and health management.

    Methods

    From July to September 2020, 8 villages and towns in Anqing City, Anhui Province were randomly selected by multi-stage random sampling. The EQ-5D-3L scale was used to calculate the EQ-5D health utility value using the latest conversion value set of the EQ-5D-3L scale based on Chinese population preference, and the visual simulation scale score (EQ-VAS) was used to evaluate the quality of life.Descriptive analysis, t-test, analysis of variance and multiple linear regression analysis were used to analyze the influencing factors.

    Results

    EQ-5D health utility value was 0.956 ± 0.062 and EQ-VAS score was 69.54 ± 13.01.The main health problems in patients with hypertension were pain/discomfort(47.3%) and anxiety/depression (29.5%).Multivariate analysis showed that age, education level, monthly average income, history of hypertension, alcohol consumption, exercise, salt intake, fat intake, gender and mental stress were the influencing factors of EQ-5D. Gender, age, education level, monthly average income, drinking, exercise, sleep time and mental stress were the influencing factors of EQ-VAS score.

    Conclusion

    The EQ-5D health utility value of patients with hypertension in rural areas of Anqing is at the average level, and the EQ-VAS score is on the low side.We should focus on women and the elderly, improve the living standards and medical services of grass-roots rural residents, strengthen psychological counseling and health education, promote a healthy lifestyle and improve the quality of life.

    Systemic lupus erythematosus (SLE) is a chronic multisystem diseases with rapid development and poor prognosis. Children have a more serious condition than adults and they also have serious psychological disorders. This paper reviews the relevant literature at home and abroad to sort out the psychological status and influencing factors of children with systemic lupus erythematosus (cSLE) in order to explore the necessity of psychological intervention for cSLE patients.
