连续硬膜外麻醉用于无痛分娩的临床效果

摘要/Abstract

摘要： 目的

探讨连续硬膜外麻醉在无痛分娩中的临床效果。

方法

选择初产妇200例，根据分娩方法分为硬膜外麻醉组（观察组）和自然分娩组（对照组）各100例。观察两组产妇疼痛等级分布、产程时间、分娩方式和不良妊娠结局情况。

结果

观察组产妇的镇痛有效率明显优于对照组，第一产程明显短于对照组，中转剖宫产率、阴道助娩率、产后出血发生率均低于对照组，自然分娩率高于对照组，差异均有统计学意义（P< 0.05）。两组第二及第三产程时间、胎儿窘迫、新生儿窒息发生率差异无统计学意义（P> 0.05）。

结论

连续硬膜外麻醉镇痛效果显著，可缩短产程，改善母婴结局，值得在临床推广应用。

Abstract: Objective

To explore the application of continuous epidural anesthesia in painless labor.

Methods

200 primiparas who met the research conditions were chosen and randomly divided into continuous epidural anaesthesia group （observation group） and guidance instrument group （control group）， with 100 cases in each group. The pain， total labor process time， delivery mode and adverse pregnancy outcomes were compared between the two groups.

Results

The analgesic effect of the observation group was better than that of the control group. The first stage time of labor of the control group was much shorter than that of the observation group. The forceps delivery rate and cesarean section rate and postpartum hemorrhage rate of the observation group were significantly lower than those of the control group. The natural delivery rate of the observation group was significantly higher than that of the control group， and the differences were statistically significant （P< 0.05）. The differences in the second and the third stage time of labor， the rate of fetal distress and neonatal asphyxia of two groups were not statistically significant （P> 0.05）.

Conclusion

Continuous epidural anesthesia can effectively relieve pain， shorten the labor process and improve maternal and infant outcomes， which is worthy of wide clinical application.
