图11例侵袭性气管支气管曲霉菌病患者诊疗过程中主要检查结果A为入院时胸部CT示双肺纹理普遍增粗,沿双肺支气管分布的点片状渗出病灶;B为住院第6天,双肺沿支气管分布的渗出病灶增多,部分区域见小空洞形成;C为抗真菌治疗1月后复查胸部CT双肺病灶减少,右上肺空洞消失;D、E为发病后5个月余,支气管CT成像示气管上段局限性狭窄(箭头),气管镜检查见气管上段重度狭窄;F为发病后4年余,随访查肺功能示中度阻塞性肺通气功能下降。
图21例侵袭性气管支气管曲霉菌病患者气管镜下表现及病理组织特点A为气管镜下见气道表面附着大量黄白色伪膜样坏死物、黏膜糜烂、触之易出血;B为支气管黏膜组织活检病理片见坏死物中曲霉菌菌丝及孢子(HE, × 200);C为支气管内膜坏死物中可见炎性渗出物,较多嗜酸性粒细胞浸润(箭头)(HE, × 200)。
