先天性心脏病患儿生长激素-胰岛素样生长因子轴的变化

摘要： 目的

研究危重程度不同的先天性心脏病患儿生长激素（GH）、胰岛素样生长因子-1（IGF-1）及胰岛素样生长因子结合蛋白-3（IGFBP-3）的变化。

方法

对53例先天性心脏病（CHD）患儿（分为低危组，中危组及高危组）及30例健康儿童静脉穿刺采血2 ml，送产筛检验科完成生长激素、胰岛素样生长因子-1及胰岛素样生长因子结合蛋白-3的测定。

结果

53例CHD组患儿血液中GH、IGF-1、IGFBP-3含量均有下降。3组CHD患儿平均出生体重、平均入院体重、入院时血液中GH、IGF-1及IGFBP-3含量不同。低、中危组和高危组患儿的平均出生体重、平均入院体重、入院时血液中GH、IGF-1、IGFBP-3含量差异均具有统计学意义（P< 0.05）。

结论

危重程度不同的CHD患儿的GH、IGF-1及IGFBP-3含量存在明显差别，尤其是高危组患儿；高危组患儿存在宫内发育迟缓可能。

Abstract: Objective

To investigate the change of growth hormone insulin-like growth factor（GH-IGF） axis in the children with congenital heart disease（CHD）.

Methods

The serum growth hormone（GH）， insulin-like growth factor-1（IGF-1） and insulin-like growth factor binding protein-3（IGFBP-3） in fifty-three children with CHD（Low risk group/medium risk group and high risk group）and 30 normal children were measured.

Results

The levels of serum GH， IGF-1 and IGFBP-3 were decreased in the patients with CHD； The birth weight，the hospital weight，the levels of serum GH， IGF-1 and IGFBP-3 were all different in the three groups. The birth weight，the hospital weight，the levels of serum GH， IGF-1 and IGFBP-3 in the low risk group and medium risk group were significantly higher than in the high risk group.

Conclusion

The levels of serum GH， IGF-1 and IGFBP-3 were significantly different in different groups， especially in the high risk group. The high risk group may have intrauterine growth retardation.
