风心病心脏瓣膜置换术后心功能、心肌酶谱水平及临床意义

Detection and clinical significance of cardiac function and myocardial enzymes in 63 patients with rheumatic heart disease after cardiac valve replacement

摘要: 目的

研究风心病心脏瓣膜置换术后心功能、心肌酶谱水平及临床意义。

方法

选取2019年1月至2020年12月在我院进行心脏瓣膜置换术的63例风心病患者进行研究。记录手术前1 d,术后1 d、7 d、14 d患者肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)、天门冬氨酸氨基转移酶(AST)、α-羟丁酸脱氢酶(α-HBD)、乳酸脱氢酶(LDH)等心肌酶谱指标的水平变化情况与射血分数(EF)、缩短分数(FS)等心功能指标的变化情况。

结果

手术前1 d,术后1 d、7 d、14 d患者的心肌酶谱水平比较,差异有统计学意义(P< 0.05);且术前1 d患者的AST、α-HBD、LDH、CK、CK-MB水平各为(53.21 ± 5.52)、(172.06 ± 20.34)、(161.17 ± 21.36)、(282.95 ± 32.14)、(16.54 ± 1.76)IU/L,明显低于术后1 d的(151.24 ± 16.75)、(413.58 ± 45.72)、(384.52 ± 36.27)、(657.83 ± 90.15)、(51.97 ± 6.23)IU/L,术后7 d的(116.93 ± 15.78)、(283.75 ± 30.92)、(265.63 ± 29.71)、(332.17 ± 35.74)、(30.24 ± 3.70)IU/L,差异有统计学意义(P< 0.05)。术后随着时间的延长,患者AST、α-HBD、LDH、CK、CK-MB水平均逐渐降低,差异有统计学意义(P< 0.05)。手术前1 d,术后1 d、7 d、14 d患者的心功能比较,差异有统计学意义(P< 0.05);术后14 d患者FS水平为(23.19 ± 2.15)%,明显高于术后1 d的(22.21 ± 2.09)%与术后7 d的(22.95 ± 2.13)%,差异有统计学意义(P< 0.05)。术后随着时间的延长,患者EF水平均逐渐升高,差异有统计学意义(P< 0.05)。

结论

风心病心脏瓣膜置换术后患者的心肌酶谱各指标与心功能的变化情况均能够作为判断手术疗效的依据。

Abstract: Objective

To study the cardiac function, myocardial enzyme levels and clinical significance of 63 patients with rheumatic heart disease after heart valve replacement.

Methods

Sixty-three patients with heart valve replacement in our hospital from January 2019 to December 2020 were selected for the study. The CK, CK-MB, AST, α-HBD, LDH, EF and FS index changes of the patients were recorded and compared 1 day before operation, 1 day after operation, 7 days after operation and 14 days after operation.

Results

There were significant differences in the levels of myocardial enzymes in patients 1 day before operation, 1 day after operation, 7 days after operation and 14 days after operation (P< 0.05); The AST, α-HBD, LDH, CK and CK-MB were (53.21 ± 5.52)IU/L,(172.06 ± 20.34)IU/L,(161.17 ± 21.36)IU/L,(282.95 ± 32.14)IU/L,(16.54 ± 76) IU /L, and they were significantly lower than those of the control group(151.24 ± 16.75)IU/L,(413.58 ± 45.72)IU/L,(384.52 ± 36.27)IU/L,(657.83 ± 90.15)IU/L,(51.97 ± 6.23)IU/L,7 days after operation (116.93 ± 15.78)IU/L,(283.75 ± 30.92)IU/L,(265.63 ± 29.71)IU/L,(332.17 ± 35.74)IU/L,(30.24 ± 70) IU/L, and the difference was statistically significant (P< 0.05).With the extension of time, the AST, α-HBD, LDH, CK and CK-MB decreased gradually, and the difference was statistically significant (P< 0.05). There were significant differences in cardiac function among patients 1 day before operation, 1 day after operation, 7 days after operation and 14 days after operation (P< 0.05); the level of FS was (23.19 ± 2.15)%, significantly higher than(22.21 ± 2.09)% and (22.95 ± 2.13)%, the difference were statistically significant (P< 0.05). After the operation, the EF level of the patients increased gradually with the extension of time, and the difference was statistically significant (P< 0.05).

Conclusion

The changes of myocardial enzymes and cardiac function in patients with rheumatic heart disease after valve replacement can be used as the basis for judging the curative effect.
