大剂量枸橼酸钠在连续静脉-静脉血液滤过治疗脓毒症患者中的应用价值

The application value of high-dose sodium citrate in continuous veno-venous hemofiltration in the treatment of patients with sepsis

摘要: 目的

探讨大剂量枸橼酸钠在连续静脉-静脉血液滤过(continuous veno-venous hemofiltration,CVVH)治疗脓毒症患者中的应用价值。

方法

以接受CVVH治疗的60例脓毒症患者为对象,按照随机数字表法分为对照组(枸橼酸钠初始剂量为1.3 × 血流速度)与观察组(枸橼酸钠初始剂量为1.5 × 血流速度)。对比两组患者血滤器相关压力参数、枸橼酸钠抗凝参数及不良事件发生情况。

结果

CVVH治疗4 h时两组滤器前压、跨膜压、滤压降升高,观察组跨膜压、滤压降水平低于对照组,差异有统计学意义(P< 0.05),两组滤器前压差异无统计学意义(P> 0.05)。CVVH治疗2 h及4 h时观察组滤器后钙离子(Ca2+)、枸橼酸钠相对速率水平均高于对照组,而滤器后Ca2+未达标率低于对照组(P< 0.05)。治疗后两组Ca2+、钠离子(Na+)、碳酸氢根离子(HCO3-)、血小板计数(platelet count,PLT)水平未见明显变化,两组间差异无统计学意义(P> 0.05),两组活化部分凝血酶原时间(activated partial prothrombin time,APTT)均升高(P< 0.05),但组间差异无统计学意义(P> 0.05)。观察组出现1例(3.33%)管路/滤器内凝血,对照组6例(20.00%),两组差异有统计学意义(χ2= 4.043,P= 0.044)。

结论

CVVH治疗脓毒症期间给予高剂量的枸橼酸钠对控制血滤器压力相关参数,减少血栓相关并发症具有积极意义,且未增加代谢性碱中毒、高钠血症等的发生。

Abstract: Objective

To investigate the application value of high-dose sodium citrate in the treatment of sepsis by continuous veno-venous hemofiltration (CVVH).

Methods

Sixty patients with sepsis treated by CVVH were divided into control group (initial dose of sodium citrate was 1.3 × blood flow velocity) and observation group (initial dose of sodium citrate was 1.5 × blood flow velocity). The relative pressure parameters of hemofilters,sodium citrate anticoagulation parameters and the occurrence of adverse events were compared between the two groups.

Results

The pre-filter pressure,transmembrane pressure,and filtration pressure drop of the two groups increased after 4 hours of CVVH treatment. The level of transmembrane pressure and filtration pressure drop in the observation group was lower than that of the control group (P< 0.05). There was no difference in pre-filter pressure between the two groups (P> 0.05). After CVVH treatment for 2 h and 4 h,the relative rates of Ca2+and sodium citrate after the filter in the observation group were higher than those in the control group, while the rate of Ca2+failure after the filter was lower than that in the control group (P< 0.05). After treatment,the levels of calcium (Ca2+), sodium (Na+),bicarbonate (HCO3-) and platelet count (PLT) in the two groups did not change significantly,and there was no significant difference between the two groups (P> 0.05). The levels of activated partial prothrombin time (APTT) in the two groups increased (P< 0.05), but there was no difference between the two groups (P> 0.05). There was 1 case (3.33%) in the observation group (3.33%) with coagulation in the tube/filter,and 6 cases (20.00%) in the control group. There was a significant difference between the two groups (χ2= 4.043,P= 0.044).

Conclusion

High-dose sodium citrate during CVVH treatment of sepsis has positive significance in controlling hemofilter pressure-related parameters and reducing the occurrence of thrombosis-related complications,and does not increase the occurrence of metabolic alkalosis and hypernatremia.
