    Allicin attenuates oxidized low density lipoprotein-induced macrophage pyroptosis

    Objective

    To investigate the inhibitory effect of allicin on oxidized low density lipoprotein (ox-LDL)-triggered macrophage pyroptosis and the underlying molecular mechanisms.

    Methods

    RAW264.7 macrophages were subjected to treatment with allicin (12.5, 25, 50 and 100 mg/L) and diphenyleneiodonium (DPI, 5 μmol/L) initially for 1 h and later for 24 h using 100 mg/L ox-LDL. Cell death degree and cell viability were detected by TUNEL detection kit and MTT assay, respectively. The levels of interleukin-1β (IL-1β) and IL-18 in media were measured using ELISA kits. Lactic dehydrogenase (LDH) activity in media, the activities of caspase-1, superoxide dismutase (SOD), and the levels of reactive oxygen species (ROS), malonaldehyde (MDA) in cells were evaluated by the corresponding detection kits. The nucleotide-binding oligomerization domain-like receptor protein 3 (NLRP3) protein expression was measured using Western blotting.

    Results

    Similar to DPI (an oxidative stress inhibitor), allicin inhibited ox-LDL-triggered macrophage pyroptosis in a dose-dependent manner, as evaluated by the increase in cell viability and the decrease in LDH release, TUNEL staining positive cells and IL-1β, IL-18 secretion. Additionally, like DPI, allicin suppressed obviously the upregulation of NLRP3 and caspase-1 activation triggered by ox-LDL. Furthermore, allicin significantly suppressed ox-LDL-induced oxidative stress, as assessed by the increased activity of SOD and the decreased levels of ROS and MDA.

    Conclusion

    Allicin can suppress ox-LDL-triggered macrophage pyroptosis at least in part by suppressing oxidative stress and attenuating the activation of NLRP3-caspase-1 pathway.

    Study on the antiepileptic mechanism of oxcarbazepine based on multi-group data

    Objective

    To explore the relationship between oxcarbazepine target gene and epileptic gene based on multi-group data, and to explore the antiepileptic mechanism of oxcarbazepine.

    Methods

    The target gene of oxcarbazepine was searched by DrugBank database. Magama software and integrated single cell sequencing data were used to verify that there was significant association between oxcarbazepine target gene and epileptic pathogenic gene at GWAS level. Fisher test was used to verify that there was significant genetic overlap between oxcarbazepine target gene and rare variant epileptic risk gene. R software was used to enrich oxcarbazepine target gene. Gene ontology function enrichment analysis and Kyoto gene and genome encyclopedia signal pathway enrichment analysis were carried out based on DAVID database.

    Results

    A total of 25 kinds of oxcarbazepine drug targets were obtained, including sodium channels, cytochrome P450 family members, aldosterone reductase family members, serum albumin and carbonyl reductase family members. Through the gene set analysis of epileptic GWAS data, there is a significant association between oxcarbazepine target gene and epileptic pathogenic gene, and there is a significant "overlap" between oxcarbazepine target gene and epileptic risk gene caused by single gene mutation, which is mainly voltage-gated sodium channel. The main types of neurons enriched by oxcarbazepine were hypothalamic GABA neurons and medium spiny neuron. The analysis of targeted enrichment pathway showed that the drug targets of oxcarbazepine were significantly enriched in the sodium channels of the central nervous system.

    Conclusion

    Based on bioinformatics integration analysis, oxcarbazepine may play an antiepileptic effect by acting on sodium channel genes in hypothalamic GABA neurons and medium spiny neuron.

    Study on the effect of MF59 and glycerol-3-phosphate compound adjuvant on the humoral immune effect of influenza A H1N1 antigen

    Objective

    To prepare the combined adjuvant MF59 and glycerol-3-phosphate(G3P), and to explore its effect on influenza A H1N1 vaccine.

    Methods

    The G3P adjuvant and MF59 were mixed as a composite adjuvant, and it was mixed with H1N1 influenza antigen alone or as a composite adjuvant to prepare a vaccine. Fifty mice were divide into five groups: blank control group, simple influenza A H1N1 antigen group, A H1N1 influenza antigen + MF59 adjuvant group, A H1N1 influenza antigen group + G3P adjuvant group, and A H1N1 influenza antigen group + G3P adjuvant group H1N1 influenza antigen + MF59 and G3P compound adjuvant group. Intraperitoneal immunization was performed twice on 0 and 28th days. After immunization, the mice were observed whether they had adverse reactions. Blood was collected on the 28th and 42nd days after the initial immunization, the level of anti-H1N1 IgG antibody was detected by ELISA, the titer of hemagglutination inhibitor was detected by hemagglutination inhibition test, and the protective effect of humoral immunity was evaluated by the pseudovirus neutralization test.

    Results

    There were no adverse reactions in each group of mice. The level of anti-H1N1 IgG induced by intraperitoneal immunization in the MF59 and G3P composite adjuvant group was significantly higher than that of the H1N1 influenza antigen group alone (P< 0.05), the MF59 adjuvant influenza antigen group (P< 0.05) and G3P adjuvant influenza antigen group (P< 0.05), combined adjuvant group hemagglutination inhibitory antibody and pseudovirus neutralizing antibody titers were also significantly higher than the traditional MF59 adjuvant influenza antigen group and G3P adjuvant influenza antigen group (P< 0.05).

    Conclusion

    The protective effect of MF59 and G3P compound adjuvant influenza A H1N1 vaccine is the best, which mainly acts by inducing the production of high titers of IgG, increasing the titer of hemagglutination inhibitory antibodies and neutralizing antibodies. This research provides an experimental basis for the development of a new type of influenza A H1N1 vaccine for humans.

    Effect of Chaiqinshuangjie decoction on Rho/ROCK pathway in rats with lung injury induced by acute pancreatitis

    Objective

    To study the effect of Chaiqinshuangjie decoction on lung tissue and Rho/ROCK pathway in rats with lung injury induced by acute pancreatitis.

    Methods

    Seventy-two rats were randomly divided into sham operation group, lung injury induced by pancreatitis group, Chaiqinshuangjie decoction low, middle and high dose groups and fasudil group, with 12 rats in each group. Chaiqinshuangjie decoction low, middle and high dose groups were given 2, 4 and 8 mL/kg Chaiqinshuangjie decoction by gavage, twice a day, and fasudil group was intraperitoneally injected with fasudil (10 mg/kg), once a day, for 5 days. The levels of arterial partial pressure of oxygen (PaO2) and partial pressure of carbon dioxide (PaCO2), the levels of myeloperoxidase (MPO), cyclooxygenase-2 (COX-2) and inducible nitric oxide synthase ((iNOS) in lung tissue, and the levels of RhoA, ROCK2 mRNA and protein in lung tissue were measured. Pulmonary histopathology was examined and scored by hematoxylin-eosin (HE) staining.

    Results

    Compared with the rats in the lung injury induced by pancreatitis group, the blood PaO2level increased significantly (P< 0.05), the blood PaCO2level and the lung tissue MPO, COX-2, iNOS, RhoA, ROCK2 mRNA and protein levels decreased significantly in the Chaiqinshuangjie decoction group (P< 0.05), and the lung histopathological score decreased significantly (P< 0.05).

    Conclusion

    Chaiqinshuangjie decoction can improve arterial blood gas index and pathological changes of lung tissue in rats with lung injury induced by acute pancreatitis, reduce the level of inflammation and oxidative injury of lung tissue, and its mechanism may be related to the regulation of Rho/ROCK pathway.

    Correlation analysis of18F-FDG PET/CT metabolic parameters with clinical pathological characteristics in colorectal cancer

    Objective

    To explore the correlation between18F-FDG PET/CT metabolic parameters and clinical pathological characteristics of colorectal cancer.

    Methods

    The clinicopathological data of 43 patients with colorectal cancer who underwent preoperative18F-FDG PET-CT examination were retrospectively collected, and the maximum standardized intake (SUVmax), average standardized intake (SUVmean), tumor metabolism volume (MTV), and sugar fermentation (TLG)of colorectal cancer were collected. The differences of metabolic parameters between different clinicopathological characteristics were compared, and the correlation between metabolic parameters and the clinicopathological characteristics was analyzed.ROC curve was drawn to evaluate the diagnostic efficacy of metabolic parameters of PET/CT in evaluating T stage.

    Results

    There were significant differences in SUVmax, SUVmean, MTV and TLG between tumor maximum diameter and T stage (P< 0.05);The difference of SUVmaxand SUVmeanin differentiation degree was statistically significant (P< 0.05); SUVmax, SUVmean, MTV and TLG were positively correlated with maximum tumor diameter and T stage (P< 0.05);SUVmaxand SUVmeanwere negatively correlated with the degree of differentiation (P< 0.05);ROC curve showed that the area under curve of SUVmax, SUVmean, MTV and TLG in differentiating T3 and T4 of colorectal cancer patients AUC thresholds were 0.734, 0.696, 0.78, 0.829 and 15.05, 0.90, 9.93, 79.70, respectively. The sensitivity and specificity of SUVmax, SUVmean, MTV and TLG for T3 and T4 stages of colorectal cancer were 83.3%, 69.6%, 83.3%, 65.2%, 91.3%, 61.1% and 91.3%, 61.1%, respectively.

    Conclusion

    The semi quantitative metabolic parameters SUVmax, SUVmean, MTV and TLG of18F-FDG PET/CT have a certain correlation with the size, differentiation and T stage of primary colorectal cancer, reflect some pathological characteristics of the tumor to a certain extent, and help to judge the characteristics and malignancy of the tumor.

    Objective

    To analyze the menstrual conditions, recurrence factors and pregnancy outcomes of patients after hysteroscopic treatment.

    Methods

    A retrospective study was conducted on the clinical data of 169 patients with transcervical resection of adhesions (TCRA). The postoperative menstruation and pregnancy of these patients were followed-up. Chi-squared test and One-Way ANOVA analysis were used to evaluate the factors that might affect postoperative pregnancy.

    Results

    121 patients had improved menstruation and didn't recur (71.60%). 66 of the 169 patients had pregnancies (39.05%). Among them, the pregnancy rate of mild IUA was 56.25%, and the rate from the moderate to severe IUA was 37.91%.

    Conclusion

    Cold knife surgery, postoperative use of physical barrier and artificial cycle drugs can effectively improve menstruation or prevent IUA recurrence. The surgical method of isolating adhesions is an important factor affecting the pregnancy rate after surgery.

    Objective

    To investigate the relationship between 25-hydroxyvitamin D[25(OH)D] and depression in elderly patients with Type 2 diabetes mellitus (T2DM) complicated with hypertension.

    Methods

    The clinical data of 172 elderly T2DM patients with hypertension were collected, including basic information, past history, health-related behaviors, 25(OH)D, triglyceride (TG), total cholesterol(TC), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), fasting blood glucose (FBG), and glycosylated hemoglobin (HbA1c).Using the Hamilton Depression Scale (HAMD), the subjects were divided into T2DM combined with hypertension and depression group (GROUP A) and T2DM combined with hypertension group (GROUP B) according to the HAMD-24 score. Pearson correlation analysis was used to evaluate the correlation between depression and 25(OH)D and other clinical data, and binary logistic regression was used to analyze the correlation between depression and multiple factors in elderly T2DM patients with hypertension.

    Results

    1. Among the 172 T2DM patients with hypertension, there were 28 (16.3%) patients with severe insufficient 25(OH)D, 84 (48.8%) patients with insufficient 25(OH)D, 40 (23.2%)patients with absent 25(OH)D, and only 20 (11.5%) patients with adequate 25(OH)D. 2. There were statistically significant differences between group A and Group B in sunbathing, physical exercise, 25 (OH) D, HbA1c and HDL-C (P< 0.05). The levels of 25 (OH) D and HDL-C in group A were lower than that in group B, while HbA1c was higher than that in group B. 3. 25 (OH) D, sunbathing, physical exercise and HDL-C were negatively correlated with depression (rS= -0.532,rS= -0.228,rS= -0.315,rS= -0.213,P< 0.05); However, HbA1c was positively correlated with depression (rS= 0.338,P< 0.05). 4. Low 25 (OH) D, high HbA1c value, less physical exercise time and less sunbathing are independent risk factors for depression in elderly T2DM patients with hypertension.

    Conclusion

    25(OH)D deficiency is common in elderly T2DM patients with hypertension. 25(OH)D level is associated with depression in elderly T2DM patients with hypertension and the higher the level of 25(OH)D is, the less likely the patients are to suffer from depression.

    The expression and clinical significance of EZH2 in AML1/ETO-positive acute myeloid leukemia

    Objective

    To investigate the expression and clinical significance of enhancer of zeste homolog 2 (EZH2) in AML1/ETO fusion gene (AML1/ETO)-positive acute myeloid leukemia (AML).

    Methods

    From January 2015 to January 2018, 40 AML1/ETO-positive AML patients admitted to the Department of Hematology in our hospital were selected as the AML1/ETO-positive group. In addition, 40 AML1/ETO-negative AML patients during the same period were selected as the control group. Real-time fluorescence quantitative PCR was used to detect the expression level of EZH2 mRNA in bone marrow mononuclear cells (BM-MNCs). After 3 years of follow-up, the Kaplan-Meier method was used to analyze the relationship between EZH2 mRNA expression and the prognosis of AML1/ETO-positive AML patients, and Cox regression analysis was used to analyze the risk factors that affected the relapse-free survival and overall survival of AML1/ETO-positive AML patients.

    Results

    The expression level of EZH2 mRNA in BM-MNCs in the AML1/ETO positive group was significantly higher than that in the control group (P< 0.05). The expression level of EZH2 mRNA in BM-MNCs of AML1/ETO-positive AML patients was related to white blood cell count, proportion of peripheral blood immature cells, extramedullary infiltration, prognosis of karyotype, and efficacy of first induction therapy (P< 0.05), and was positively correlated with the white blood cell count and the proportion of immature cells in peripheral blood (P< 0.05). The Kaplan-Meier curve showed that the 3-year overall survival rate of the EZH2 mRNA low expression group was significantly higher than that of the EZH2 mRNA high expression group (P< 0.05). Cox regression analysis showed that the efficacy of the first induction treatment and EZH2 mRNA expression were independent risk factors that affected the relapse-free survival and overall survival of AML1/ETO-positive AML patients (P< 0.05).

    Conclusion

    The expression of EZH2 mRNA is high in AML patients with positive AML1/ETO, which can be used as a biomarker to assess the prognosis of patients.

    Meta analysis of the effect of early rehabilitation exercise on patients with acute myocardial infarction after PCI

    Objective

    To systematically evaluate the effect of early rehabilitation exercise on patients with acute myocardial infarction after percutaneous coronary intervention.

    Methods

    The randomized controlled trials on the impact of early exercise rehabilitation on the quality of life of patients after PCI in several Chinese and English databases such as VIP, Wanfang Data, CNKI, SinoMed, PubMed, Web Of Science, EMbase and Cochrane Library were searched by computer. The retrieval time was up to August 2021, and revman 5.4 software was used for Meta-analysis.

    Results

    A total of 14 literatures were included. The results of Meta-analysis showed that compared with the control group, the left ventricular ejection fraction in the experimental group increased (MD= 5.18,95%CI:4.13 ~ 6.22,P< 0.001); The 6 minute walking distance was prolonged (MD= 58.65,95%CI:24.12 ~ 93.19,P< 0.001); The length of hospital stay was shortened (MD= -2.11,95%CI:-2.26 ~ -1.97,P< 0.001); The incidence of postoperative complications decreased(OR= 0.17,95%CI:0.10 ~ 0.27,P< 0.001).

    Conclusion

    Current evidence shows that early rehabilitation exercise can promote the recovery of cardiac function, shorten hospital stay and reduce the incidence of postoperative complications in patients with acute myocardial infarction after PCI; The above conclusions still need to be verified by high-quality research so as to provide more evidence-based basis for early rehabilitation exercise after PCI.

    Objective

    To explore the application value of Internet + remote nursing platform in continuous care after permanent enterostomy during COVID-19.

    Methods

    Seventy-six patients undergoing permanent enterostomy for colorectal cancer in the Department of General Surgery from June 2020 to June 2021 were selected and divided into experimental group and control group according to randomization principlewith38 patients in each group. Conventional nursing was used in the control group, while the Internet platform about nurses home service, remote nursing education, remote video guidance and other nursing methods were used in the experimental group on the basis of conventional nursing. Three months after operation, the two groups were compared in terms of the treatment, complications, ESCA score, SF-36 score and ostomy health knowledge score.

    Results

    The rates of returnto hospital treatment were 13.15% in the experimental group and 52.63% in the control group, and the difference between the two groups was significant (P< 0.05). The incidence of complications in the experimental group was lower than that in the control group, and the difference was statistically significant (P< 0.05). There was no significant difference between the two groups in ESCA, SF-36 score and ostomy health knowledge score on discharge day (P> 0.05), but the three scores were significantly increased after the implementation of Internet + remote nursing, and there were statistical differences between the two groups (P< 0.05).

    Conclusion

    Internet + remote nursing mode can effectively reduce the rate of return to hospital treatment, strengthen the self-care ability of patients, and better avoid the occurrence of various complications to reduce the complications related to ostomy nursing, and improve the quality of life of patients.

    Hospitalization expenses and influencing factors of elderly chronic diseases based on BP neural network model

    Objective

    To investigate the hospitalization costs of six common chronic diseases in the elderly, to explore the economic burden of chronic diseases in the elderly and to analyze the main factors affecting hospitalization costs, so as to provide the basis for rational formulating the cost control policies for chronic diseases in the elderly.

    Methods

    In tthis study the method of retrospective study and self-made questionnaire were used to collect the information on the front page of hospitalization medical records of the elderly aged over 60 years in a tertiary first-class hospital in Anhui Province from January 1, 2018 to December 31, 2020. The median (quartile) was used for descriptive statistical analysis, and the hospitalization expenses by single factor analysis were analyzed by non-parametric test. The sensitivity of each variable was obtained by neural network model, and the influence of each factor on hospitalization expenses was evaluated.

    Results

    The study showed that the average hospitalization cost of the elderly with chronic diseases was 10 705.8 yuan; the cost of drugs and examinations was the highest proportion of hospitalization expenses; age, urban and rural areas, medical insurance type, operation or not, hospitalization days and diseases were the influencing factors of hospitalization expenses; Among them, the most critical factors affected hospitalization expenses were hospitalization days, age and respiratory system disease.

    Conclusion

    The cost of hospitalization for chronic diseases of the elderly is high, among which drugs and examinations account for a high proportion, and the economic burden is heavy. The older the elderly are, the longer they stay in hospital, the higher the cost is. Respiratory diseases are the most expensive among common chronic diseases.

    Objective

    To investigate the status and related influencing factors of hyperhomocysteinemia (HHcy) in 607 middle-aged and elderly patients with hypertension in Yuexi County, Anhui Province, and to provide scientific basis for the prevention and treatment of cardiovascular and cerebrovascular diseases.

    Methods

    607 patients with hypertension in Yuexi County, Anhui Province were investigated by multi-stage sampling method from June to September 2020. The subjects were divided into hyperhomocysteinemia (HHcy group) and non-hyperhomocysteinemia (non-Hcy group) according to plasma homocysteine (Hcy) levels. Blood pressure, smoking history and other information were obtained through physical examination and questionnaire survey. The Hcy and blood lipid levels of the subjects were obtained through laboratory testing and SPSS 16.0 statistical software was used for data processing and statistical analysis.

    Results

    A total of 607 middle-aged and elderly patients with hypertension were included in the study. The mean level of Hcy was (17.41 ± 16.63) μmol/L, and the mean level of Hcy in non-HHcy group was (11.59 ± 2.29) μmol/L. The average level of plasma Hcy in HHcy group was (24.39 ± 22.66) μmol/L. 276 patients had HHcy with a prevalence of 45.47%, including 179 males with a prevalence of 57.01% and 97 females with a prevalence of 33.11%. There were statistically significant differences between the non-HHcy group and the HHcy group in gender, diastolic blood pressure (DBP), systolic blood pressure (SBP), high-density lipoprotein cholesterol (HDL-C), total bilirubin (TBil) and creatinine (P< 0.05). Multivariate logistic regression analysis showed gender (OR= 0.512, 95%CI: 0.353 ~ 0.742), DBP (OR =1.012, 95%CI: 1.001 ~ 1.023), creatinine (OR= 1.018, 95%CI:1.009 ~ 1.027) and HDL-C (OR= 0.593, 95%CI: 0.354 ~ 0.993) were the influencing factors of HHcy.

    Conclusion

    The prevalence of HHcy is higher in males than in females. Gender, blood pressure, blood lipid and creatinine levels are risk factors for HHcy.

    Dapagliflozin is a newer type of hypoglycemic agent of sodium-glucose transporter 2 inhibitors (SGLT-2i), which lowers blood sugar by promoting the excretion of urine sugar. There is a large population of chronic kidney disease (CKD) in China, and the treatment options are limited. Dapagliflozin is not only a good hypoglycemic drug, but it can also reduce adverse cardiovascular events and delay diabetic nephropathies (DN). Renal benefits have also been observed in CKD patients with non-diabetic nephropathy. This article intends to summarize the research progress of dapagliflozin in the treatment of CKD, explore its possible mechanism of action and safety and provide guidance for clinical treatment.

    Most patients with malignant tumors have disturbance of blood coagulation, and D-dimer is one of many coagulation and fibrinolysis indexes that have the closest correlation with the disease progression of lung cancer patients. CEA is an acidic glycoprotein on the surface of cancer cells derived from the differentiation of endodermal cells,which is widely used in non-small cell lung cancer (NSCLC) and one of the earliest tumor markers for lung cancer diagnosis. SCC-Ag is a glycoprotein present in the cytoplasm of squamous cell carcinoma. Many studies have shown that SCC-Ag has a high specificity for the diagnosis of NSCLC, and its level is related to tumor progression. Combined detection of multiple tumor markers has achieved some results in improving the diagnosis of lung cancer. However, the combination of coagulation indicators and tumor markers is rarely used to evaluate the clinical significance of the diagnosis of NSCLC. In order to further clarify the relationship about whether the combination of the two can improve the diagnostic value of NSCLC and serve as a new way to diagnose NSCLC, in this paper we review the research progress of the relationship between D-dimer, CEA, SCC-Ag and NSCLCby searching a large number of literatures..
