    This paper presents an update of the 2015 European Society of Cardiology (ESC) Guidelines for the management of patients with ventricular arrhythmias and the prevention of sudden cardiac death, which was issued by ESC in August, 2022, with many new contents and updated recommendations. The guidelines have added recommendations for public basic life support, genetic testing, risk assessment, and changed recommendations for the treatment of ventricular arrhythmias, to preferably guide clinical practice.

    Postoperative atrial fibrillation (POAF) is a common complication of cardiac surgery, with an incidence of 20% ~ 40%. Typical features include onset mostly at 2 ~ 4 days after surgery, fleeting episode, and self-limitation. POAF may cause hemodynamic instability and increase the risk of stroke, thus increasing the length and cost of stay in hospital. In addition, a recent observational cohort study has suggested that patients who developed POAF after cardiac valve surgery still has a great long-term risk of thromboembolism. A series of studies have shown that atrial inflammation is the central link in the pathogenesis of POAF, and its related electrical remodeling, calcium handling abnormalities, fibrosis, oxidative stress, and/orautonomic nervous system activation jointly promote the onset of POAF. Combined with a series of results of our research group, the proarrhythmic mechanisms of POAF are discussed in this review.

    Effects of simvastatin combined with fucoidan administration on osteogenic differentiation of BMSCs

    ObjectiveTo investigate the effect of simvastatin (SIM) combined with fucoidan(FD)administration on the middle-stage differentiation of BMSCs from SD rats to osteoblasts in the osteogenic induction environment in vitro.Methods(1) Primary cells were extracted from the bone marrow cavity of bilateral femurs and tibias of SD rats, and cultured in vitro to P3 passage for cell identification. The identification methods included cell morphology observation, flow identification, adipogenic identification and chondrogenesis identification. (2) P3 generation BMSCs were taken and divided into blank group (DMEM complete medium) and experimental group (DMEM complete culture containing different FD concentrations base). ① CCK-8 method was used to detect the cell activity of each group on 1, 2, and 3 days of culture. ② To the 7th day of culture, the total RNA in the cells was extracted, and the qPCR method was used to detect the expressions of mRNR in Alkaline phosphatase (ALP) and bone morphogenetic protein-2 (BMP-2).. ③ To the 7th day, the qualitative and quantitative analysis of ALP were carried out. Based on the above results, the administration concentration of FD was determined. (3) P3 generation BMSCs were taken and divided into control group (OM group), SIM group, FD group and SIM+FD group. ① CCK-8 method was used to detect the cell viability of each group for 1, 2, and 3 days of culture. ② The cell morphology and growth status were observed on the 7th day of culture, and photographed and recorded. ③ To the 7th day of culture, the total cells in the cells were extracted. RNA, qPCR method was used to detect the expressions of BMP-2, Type I collagen (COLⅠ), RUNX2 and ALP mRNA. ④ To the 7th day of culture, the qualitative and quantitative analyses of ALP wwere carried out.Results(1) ① Observation of cell morphology showed that cells slowly extended and stretched out during the culture process. After 3 generations, the shape of the cells was basically the same, and the fibroblast-like growth adhered to the wall. ② The detection of flow cytometry showed that CD29, CD90 and CD44 were positive, and the expression rates were 99.1%, 99.5%, and 99.4%, respectively. CD45 was negative, and the expression rate was 0.4%. ③ The identification of adipogenesis showed that during the induction process, the cells gradually became round, and lipid droplets with high refractive index and different sizes appeared in the cells, which turned red after staining with Oil Red O. ④ The identification of cartilage formation showed that cells gradually changed from long spindle to triangle or polygon. After Alcian blue staining, the cartilage matrix was blue, and the nucleus was red. (2) The administration concentrations of FD were as follows. ① In the aspect of cell viability, the viability of BMSCs increased with the culture time and the concentration of FD. Compared with the blank group, the experimental group had statistical significance (P< 0.001). ② qPCR showed that the expressions of BMP-2 and ALP mRNA in the FD group at a concentration of 1 μg/ml were higher than those in the blank group and other experimental groups, and the difference was statistically significant (P< 0.05). ③ ALP qualitative and quantitative analysis showed that when the concentration of FD was 1 μg/mL, the staining was the deepest, and the activity of ALP was the highest (P< 0.001), indicating that the concentration of FD was suitable for the growth and differentiation of BMSCs. (3) The osteopromoting effect of the combination of the two drugs was as follows. ① In the aspect of cell activity, the cell activity in SIM+FD group was significantly higher than that in OM group and other experimental groups (P< 0.001), indicating that the combination of the two drugs could promote cell proliferation; ② Cell morphology observation of cell growth showed that the number of cells was significantly higher in SIM+FD group than that in the OM group and other experimental groups; ③ qPCRshowed that the expression of bone-related gene mRNA in the SIM+FD group was higher than that in the OM group, SIM group and FD group (P< 0.001); ④ In the aspect of ALP qualitative and quantitative analysis, the quantitative results showed that the ALP staining results were the deepest in the SIM+FD group. Quantitative data also showed that the combined group had the highest ALP expression (P< 0.001).ConclusionSimvastatin combined with fucoidan administration can significantly promote the differentiation of BMSCs into osteoblasts, and increase cell activity and the expression of osteogenesis-related factors in the middle stage of differentiation.

    Effects of norcantharidin on proliferation and apoptosis of nasopharyngeal carcinoma CNE-2 cells

    ObjectiveTo study the effects of norcantharidin (NCTD) on proliferation and apoptosis of nasopharyngeal carcinoma (NPC) CNE-2 cells.MethodsCNE-2 cells were divided into control group and different doses of NCTD group, and cultured in DMEM high glucose medium containing 0, 35, 70, 140 μmol/L NCTD. The proliferation rate, clonogenesis ability and apoptosis rate of CNE-2 cells were determined by CCK-8 assay, clonogenesis assay and flow cytometry, respectively. Cyclin B mRNA expression level in CNE-2 cells was detected by real-time quantitative PCR.ResultsAt 48 h, the IC50 value of NCTD on CNE-2 cells was (89.07 ± 3.702) μmol/L. Compared with the control group, 35, 70 and 140 μmol/L NCTD groups significantly decreased cell proliferation and clonal formation ability (P< 0.05), cell cycle arrest in G2/M phase, Cyclin B expression significantly decreased (P< 0.05), and the apoptosis rate was significantly increased ((P< 0.05).ConclusionNCTD can induce the apoptosis of CNE-2 cells and inhibit the proliferation and clonogenesis of CNE-2 cells by down-regulating Cyclin B expression.

    Association between psychological stress response and IgG N-glycan among the students of a university in Shandong Province during COVID-19 epidemic

    ObjectiveTo explore the correlation between psychological stress response and immunoglobulin G (IgG) N-glycan among the students of a university in Shandong Province during the epidemic.MethodsAccording to the inclusion and exclusion criteria, 150 students from Shandong First Medical University during the epidemic were selected as the research objects. SRQ questionnaire was used to investigate the psychological stress level of the college students, and ultra-high liquid chromatography was used to detect and analyze plasma IgG N-glycans. The Mann-WhitneyUrank-sum test was used for comparison of IgG N-glycans between groups; multivariate Logistic regression analysis was used to explore the association between psychological stress and IgG N-glycans after adjusting for confounding factors; Spearman rank correlation correlation was used to analyze the correlation between IgG N-glycan level and psychological stress level.ResultsCompared with the normal group, the levels of GP4 and GP8 in the high psychological stress group were higher, and the level of G2 was lower (P< 0.05). The degree of psychological stress was positively correlated with GP3, GP4, GP8 and G0, and negatively correlated with GP12, GP14, GP17, GP18, G2, Stotal and S1 (P< 0.05). On the whole, IgG galactosylation and salivary level of acidification modification were negatively correlated with the degree of psychological stress.ConclusionThere is a correlation between psychological stress response and IgG N-glycan among the students in a Shandong university during the epidemic. The level of galactosylation and sialylation reflected by IgG N-glycans may affect the degree of psychological stress response by altering the inflammatory potency of IgG.

    Diagnostic value of craniocerebral ultrasound combined with serum lncRNA GAS5 level in neonatal brain injury

    ObjectiveTo investigate the diagnostic value of craniocerebral ultrasound combined with serum long non-coding RNA (lncRNA) growth arrest-specific transcript 5 (GAS5) in neonatal brain injury.MethodsA total of 162 high-risk neonates delivered in the Affiliated Children's Hospital of Zhengzhou University from January 2019 to September 2021 were collected as the research objects. According to the situation of brain injury, they were divided into the observation group (92 cases with brain injury) and the control group (70 cases without brain injury). All neonates underwent cranial ultrasonography. The expression level of GAS5 in serum was detected by real-time fluorescence quantitative PCR. Serum levels of brain-derived neurotrophic factor (BDNF) and glial fibrillary acidic protein (GFAP) were detected. The correlation between serum GAS5 level and BDNF and GFAP was analyzed by Pearson method. The value of serum GAS5 level in the diagnosis of neonatal brain injury was evaluated by Receiver Operating Characteristic (ROC) curve, the early diagnostic value of craniocerebral ultrasound combined with serum GAS5 in neonatal brain injury was analyzed.Results79 cases of brain injury were diagnosed by brain ultrasound in the observation group, and the positive rate of ultrasonography was 85.87%. Among them, 34 cases of intracranial hemorrhage, 22 cases of cerebral edema, and 18 cases of cerebral white matter injury were detected by ultrasonography. The incidences of intracranial hemorrhage and white matter injury in preterm infants were higher than those in full-term infants, and the incidence of cerebral edema in full-term infants was higher than that in preterm infants (P< 0.05). Compared with the control group, the relative expression level of serum GAS5 and the level of GFAP in the observation group were higher, the level of BDNF was lower (P< 0.05). Pearson method analysis showed that the expression level of serum GAS5 was negatively correlated with BDNF (P< 0.05), and positively correlated with GFAP (P< 0.05). The level of serum GAS5 in the abnormal craniocerebral ultrasound group was higher than that in the normal craniocerebral ultrasound group (P< 0.05). The accuracy of craniocerebral ultrasound combined with serum GAS5 detection in the early diagnosis of neonatal brain injury was 90.12%, the sensitivity was 94.57%, and the specificity was 84.29%, and the accuracy of brain ultrasonography combined with serum GAS5 detection in the diagnosis of severe brain injury was 84.81%.ConclusionSerum GAS5 is up-regulated in neonatal brain injury, and craniocerebral ultrasound combined with serum lncRNA GAS5 level has early diagnostic value for neonatal brain injury.

    Predictive value of MPVLR combined with hs-CRP in contrast-induced nephropathy

    ObjectiveTo explore the early predictive value of mean platelet volume/lymphocyte ratio (MPVLR) and high sensitivity C-reactive protein (hs-CRP) in contrast-induced nephropathy (CIN) after percutaneous coronary intervention (PCI) in patients with acute coronary syndrome (ACS).MethodsThe data of 495 ACS patients who underwent PCI in the Department of Cardiology of Tai’an Central Hospital from December 2020 to December 2021 were retrospectively collected. The relevant examinations of patients after admission were made and the MPVLR and hs-CRP values of patients were recorded. According to the diagnostic criteria of CIN, the patients were divided into CIN group and non-CIN group. The patients in the two groups were compared for statistical significance. The correlation between various indicators and CIN was analyzed. The receiver operator characteristic (ROC) curve of diagnostic efficacy was drawn to evaluate the diagnostic efficacy of MPVLR and hs-CRP in predicting CIN.ResultsThere was significant difference between MPVLR and hs-CRP groups (P< 0.05). The incidence of CIN was positively correlated with MPVLR (r= 0.325,P< 0.001) and hs-CRP (r= 0.332,P< 0.001). ROC curve showed that when predicting the occurrence of CIN, the threshold values of MPVLR, hs-CRP and their combined area under curve (AUC) were 0.814, 0.821 and 0.847, the sensitivity and specificity of MPVLR, hs-CRP and their combination were 83.7%, 65.0%, 67.3%, 84.3% and 89.8%, 64.6% respectively.ConclusionMPVLR and hs-CRP were positively correlated with the occurrence of CIN after PCI in patients with ACS.MPVLR and hs-CRP have strong sensitivity and specificity in the early diagnosis of CIN after PCI in patients with ACS. They can play a good predictive role and are conducive to the early diagnosis of CIN.

    Clinicopathological features of monomorphic epitheliophilic intestinal T-cell lymphoma

    ObjectiveTo summarize the clinical pathological characteristics,immunohistochemistry, molecular genetic changes, treatment and prognosis of monomorphic epitheliotropic intestinal T-cell lymphoma (MEITL).MethodsA total of 3 patients with MEITL in the Second Affiliated Hospital of Shandong First Medical University were collected from 2018 to 2022. Hematoxylin eosin staining, immunohistochemistry staining, in situ hybridization and clonal rearrangement of T cell receptor (TCR) gene were used to detect their histopathological features. The diagnosis and treatment as well as prognosis of the patients were reviewed, and their clinical pathological characteristics were analyzed.ResultsAll three patients lesions were located in the small intestine,and one of them infiltrated multisegmental small intestine, sigmoid colon, uterus and bladder. They had abdominal distension, abdominal pain, intestinal perforation and diffuse peritonitis at the time of admission, and two of them had fever. They were all treated by surgery, one patient also received chemotherapy, and all three patients died. Pathological characteristics showed that tumor cells grew diffusely and infiltrated the whole layer of the intestinal wall. The tumor cells had a single morphology, small to medium sizes, lightly stained cytoplasm, round nuclei, inconspicuous nucleoli, and exquisite chromatin. “Epitheliotropic phenomenon” were seen, together with mitotic figures and the formation of necrosis. Immunohistochemistry showed that CD3, CD8, CD56, Bcl-2 and TIA-1 were positive; CD4 was negative in 2 cases and partially positive in 1 case.CD20 was negative in 2 cases and partially weak positive in 1 case;Granzyme B was negative in 2 cases and scatteredly positive in 1 case;CD45RO was partially positive in all 3 cases;CD21, PAX-5 and CD5 were negative, the proliferation index of Ki-67 was 40% ~ 70% . In situ hybridization showed that small RNA encoded by Epstein Barr virus was negative and there was no EB virus infection.TCR rearrangement was positive in all 3 cases.ConclusionMETCL is a rare highly aggressive T-cell lymphoma, most of which is located in the small intestine, prone to intestinal perforation, with rapid progression and poor prognosis. The diagnosis should be comprehensively judged based on the combination of clinical manifestations, pathological features, immunohistochemistry, and gene detection results. The prognosis of patients with serious complications is very poor. The patients who have an early diagnosis, timely operation and good chemotherapy reaction have a relatively long survival time.

    Transanal endoscopic microsurgery (TEM) is a new surgical method which combines the advantages of anal approach, rectal endoscopic technology, laparoscopic technology and minimally invasive technology. This surgical method can enlarge the operative field of vision by six times,help the operator make more clear and accurate resection of upper and middle rectal lesions, overcome the limitations of resection of anal sphincter and pelvic bone boundary, and it is suitable for rectal adenoma, rectal polyp, etc. TEM is suitable for ct1 rectal cancer without lymph node metastasis, but there is still some controversy about the treatment of higher grade rectal cancer. Neoadjuvant radiotherapy and chemotherapy combined with TEM in the treatment of rectal cancer is being actively carried out at home and abroad, and good results have been achieved. This paper reviews the surgical instruments, methods, indications, preoperative evaluation, latest progress and controversy of TEM.

Recently, it has been found that intestinal flora is widely involved in the fat metabolism of the host and plays an important role in the development of obesity in adolescents. This discovery provides a new possible way for the treatment of adolescent obesity, and makes it possible to treat adolescent obesity by intervening in the intestinal flora.     Prostate cancer (PCa) is a common malignant tumor in men, with an increasing incidence rate and poor prognosis. Early screening can lead to early diagnosis and treatment, and significantly improve the quality of life and survival rate; MRI has proved to play an important role in PCa screening and active monitoring. However, multi parameter prostate MRI has many sequences, long examination time and high cost, which limits the popularity of prostate MRI screening. Therefore, it has become the focus of clinical attention to optimize the scanning sequence of MRI, shorten the examination time and improve the diagnostic coincidence rate. This article reviews the application progress of MRI in early screening of prostate cancer.
