膀胱副神经节瘤多期增强CT特征分析

Multiphase enhancement CT features of bladder paraganglioma

摘要: 目的

探讨膀胱副神经节瘤(bladder paraganglioma, BPG)的多期动态增强CT表现特征,以提高对该病的诊断准确率。

方法

回顾性分析经手术病理证实的8例BPG的临床及CT影像资料,CT图像主要从肿瘤部位、大小、形态、密度、强化特征等因素进行评估。

结果

本组病例中男性2例,女性6例,中位年龄为53.8岁。其中慢性高血压患者2例,尿儿茶酚胺正常,术中出现血压剧烈波动1例。CT上病灶多突向腔内生长7例,呈圆形或卵圆形,边界清晰,内部密度较均匀,5例增强扫描为中-高度强化(3例伴有迂曲供血血管),强化曲线均为速升缓降型,3例表现为轻度持续性强化。

结论

BPG多数为良性,多期CT增强扫描对其诊断有一定价值。

Abstract: Objective

To investigate the clinical and multiphase enhancement CT features of bladder paraganglioma(BPG),so as to improve its diagnostic accuracy.

Methods

The clinical and CT data of 8 BPG cases confirmed by surgical resection and pathology were retrospectively analyzed. CT images were used to evaluate tumor location, size, shape, density and enhancement characteristics.

Results

Two males and 6 females were included with average age of 53.8 years. Chronic hypertension was observed in 2 cases, preoperative urinary catecholamines were normal in the two cases. Significant fluctuant intraoperative blood pressure was observed in 1 case. Lesions of 7 cases were located in the bladder wall and grew into the lumen. All the lesions were regular in shape with smooth margin and homogeneous density in plane scan. Moderate to high enhancement was observed in 5 cases (3 cases accompanied by tortuous blood supply vessels), slight continuous enhancement in 3 case was observed with contrast-enhanced CT scan.

Conclusion

Most BPGs are benign. Multiphase dynamic enhanced CT scan is useful for the diagnosis.
