多西环素联合牙周基础治疗对牙周炎患者牙周指标的影响

Effect of doxycycline combined with periodontal basic therapy on periodontal indexes in patients with periodontitis

摘要:

目的探究多西环素联合牙周基础治疗对牙周炎患者牙周指标的影响。方法选择2019年4月至2022年4月安徽医学高等专科学校口腔医学院收治的牙周炎患者102例,按随机数字表法分为对照组(51例,牙周基础治疗)、联合组(51例,牙周基础治疗+多西环素),评估治疗效果,比较两组患者治疗前及治疗后龈沟液(gingival crevicular fluid,GCF)量、GCF中转化生长因子-β1(transforming growth factor-β1,TGF-β1)及炎性因子[肿瘤坏死因子-α (tumor necrosis factor-α,TNF-α)、白介素-6(interleukin-6,IL-6)]水平,进行牙周指标[探诊深度(probing depth,PD)、龈沟出血指数(sulcus bleeding in-dex,SBI)、菌斑指数(plaque index,PLI)]评价。结果联合组治疗总有效率高于对照组,差异有统计学意义(P< 0.05);治疗3个月后,联合组GCF量低于对照组,GCF中TGF-β1水平高于对照组,差异有统计学意义(P< 0.05);联合组GCF中TNF-α、IL-6水平低于对照组,差异有统计学意义(P< 0.05);联合组PD、PLI、SBI均低于对照组,差异有统计学意义(P< 0.05)。结论多西环素联合牙周基础治疗可显著下调牙周炎患者龈沟液内炎性因子水平,改善牙周指标,提升整体疗效。

Abstract:

ObjectiveTo investigate the effect of doxycycline combined with periodontal basic therapy on periodontal indexes in patients with periodontitis.MethodsFrom April 2019 to April 2022, 102 patients with periodontitis from the School of Stomatology of Anhui Medical College were collected. They were randomly divided into the control group (51 patients receiving periodontal basic treatment) and the combined group (51 patients receiving periodontal basic treatment + doxycycline). The therapeutic effect was evaluated and the amount of gingival crevicular fluid (GCF) , the transforming growth factor in GCF-β1 (transforming growth factor-β1,TGF-β1) inflammatory factors [tumor necrosis factor-α (tumor necrosis factor-α, TNF-α) and nterleukin-6 (IL-6) level were compared before and after treatment, and periodontal indicators [probing depth (PD), sulcus bleeding in dex (SBI) and plaque index (PLI)] were evaluated.ResultsThe total effective rate in the combination group was higher than that in the control group (P< 0.05); after 3 months of treatment, the amount of GCF in the combination group was lower than that in the control group, and the level of TGF-β1 in GCF was higher than that in the control group (P< 0.05); the levels of TNF-α and IL-6 in GCF in the post-combination group were lower than those in the control group (P< 0.05); PD, PLI and SBI in the combination group were lower than those in the control group after 3 months of treatment (P< 0.05).ConclusionDoxycycline combined with periodontal basic therapy in patients with periodontitis can significantly down-regulate the levels of inflammatory factors in gingival crevicular fluid, improve periodontal indexes, and enhance the overall curative effect.
