安庆市农村地区高血压患者生命质量研究

摘要/Abstract

摘要： 目的

探讨安庆市农村地区高血压患者的生命质量及影响因素，为提高该人群生命质量和健康管理提供依据。

方法

于2020年6—9月采用多阶段随机抽样方法在安徽省安庆市随机抽取8个乡镇，采用EQ-5D-3L量表，使用最新的基于中国人群偏好的EQ-5D-3L量表效用值换算值集计算EQ-5D健康效用值，结合可视化模拟标尺评分（EQ-VAS）评价生命质量。采用描述性分析、t检验、方差分析以及多元线性回归分析其影响因素。

结果

EQ-5D健康效用值为0.956 ± 0.062，EQ-VAS评分为69.54 ± 13.01，高血压患者存在的主要健康问题在疼痛/不适（47.3%）以及焦虑/抑郁（29.5%）两维度。此外，多因素分析显示，性别、年龄、文化程度、月平均收入、高血压病史、饮酒、运动、食盐摄入量、脂肪摄入量和精神紧张是EQ-5D健康效用值的影响因素，性别、年龄、文化程度、月平均收入、饮酒、运动、睡眠时间和精神紧张是EQ-VAS评分的影响因素。

结论

安庆市农村地区高血压患者的EQ-5D健康效用值处于平均水平，EQ-VAS评分偏低。应重点关注女性、老年人，提高基层农村居民的生活水平和医疗服务水平，加强心理疏导和健康宣教，提倡健康的生活方式，提高生命质量。

Abstract:

Objecttive：To explore the quality of life and its influencing factors of patients with hypertension in rural areas of Anqing City， so as to provide basis for improving their quality of life and health management.

Methods

From July to September 2020， 8 villages and towns in Anqing City， Anhui Province were randomly selected by multi-stage random sampling. The EQ-5D-3L scale was used to calculate the EQ-5D health utility value using the latest conversion value set of the EQ-5D-3L scale based on Chinese population preference， and the visual simulation scale score （EQ-VAS） was used to evaluate the quality of life.Descriptive analysis， t-test， analysis of variance and multiple linear regression analysis were used to analyze the influencing factors.

Results

EQ-5D health utility value was 0.956 ± 0.062 and EQ-VAS score was 69.54 ± 13.01.The main health problems in patients with hypertension were pain/discomfort（47.3%） and anxiety/depression （29.5%）.Multivariate analysis showed that age， education level， monthly average income， history of hypertension， alcohol consumption， exercise， salt intake， fat intake， gender and mental stress were the influencing factors of EQ-5D. Gender， age， education level， monthly average income， drinking， exercise， sleep time and mental stress were the influencing factors of EQ-VAS score.

Conclusion

The EQ-5D health utility value of patients with hypertension in rural areas of Anqing is at the average level， and the EQ-VAS score is on the low side.We should focus on women and the elderly， improve the living standards and medical services of grass-roots rural residents， strengthen psychological counseling and health education， promote a healthy lifestyle and improve the quality of life.
