新型冠状病毒肺炎合并心血管疾病的Meta分析

Prevalence of cardiovascular comorbidities in severe acute respiratory syndrome coronavirus 2(SARS-CoV-2): a systematic review and Meta-analysis

摘要: 目的

使用Meta分析评估严重急性呼吸综合征冠状病毒2型(severe acute respiratory syndrome coronavirus 2, SARS-CoV-2)感染患者的主要合并症患病率,以及死亡病例的合并症患病率,以找出潜在的高危人群。

方法

检索万方数据库、中国学术期刊全文数据库(CNKI)、维普中文科技期刊数据库,中国生物医学文献数据库。检索时间段为2019年12月1日至2020年5月3日。使用统计软件Open Meta Analyst version10.10进行新冠肺炎患者的基础疾病高血压、糖尿病的Meta分析,统计分析结果以森林图表示。

结果

通过对4814例新冠肺炎确诊病例的Meta分析,高血压患者占比为19.6%,糖尿病患者占比为9.3%。根据《中国心血管病报告2018概要》,高血压和糖尿病的人群患病率分别为23.2%和10.9%。将本研究数据与2018年中国心血管疾病报告中的数据进行比较,发现感染该病毒的人群中的高血压和糖尿病患病率与普通人群大致相同。而在死亡病例中,高血压患者占比达40.0%,糖尿病患者在死亡病例中占比为21.5%,60岁以上的老年人超过70.0%。

结论

不能认为高血压、糖尿病患者容易感染新冠肺炎;但高血压、糖尿病基础疾病、年龄 ≥ 60岁可使新冠肺炎患者死亡风险增加。针对这些慢性疾病患者制定公共卫生疫苗接种战略可以提高新冠肺炎患者的生存率。

Abstract: Objective

To evaluate the prevalence of major comorbidities and death cases and to identify potential high risks disease associated with severe acute respiratory syndrome type 2(SARS-CoV-2) infection by meta-analysis.

Methods

Literatures search was conducted using Wanfang Database, CNKI, CQVIP Database and Chinese Biomedical Literature Database(CBM) from 1 December 2019 to 3 May 2020. Meta-analysis of hypertension and diabetes in patients with coronavirus disease was conducted by using Open Meta-Analyst version 10.10. Forest plots were used to illustrate the prevalence of the hypertension and diabetes.

Results

A total of 4814 patients with SARS-CoV-2 infection were included. The rates of hypertension and diabetes were 19.6% and 9.3%, respectively. The prevalence rates of hypertension and diabetes were 23.2% and 10.9%, respectively according to the summary of the 2018 Report on Cardiovascular Disease in China. In comparion of our data with that of the 2018 Report, we concluded that the prevalence of hypertension and diabetes among people infected with the SARS-CoV-2 is roughly the same as that in the general population, but the morbidities of the hypertension and diabetes were 40.0% and 21.5%, respectively. More than 70.0% of patients was over 60 years old.

Conclusions

It can not be said that the patients with hypertension and diabetes are more susceptible to SARS-CoV-2 infection, but hypertension, diabetes and > 60 years old can increase the risk of the morbidities in patients with SARS-CoV-2 infection. The survival rate of patients with SARS-CoV-2 disease can be improved if a public health vaccination strategy is developed for those with these chronic diseases.
