围术期呼吸道综合管理对肺癌术后快速康复的临床意义

摘要： 目的

分析并研究围术期间快速康复理念下的呼吸道综合管理对肺癌患者术后快速康复的影响。

方法

收集2020年8月至2021年6月betway必威登陆网址第二附属医院胸外科入院且接受手术治疗的肺癌患者。根据数字奇偶法随机将患者分为试验组（行呼吸道综合管理）和对照组（行常规护理），试验组手术前接受一系列加速康复外科（enhanced recovery after surgery， ERAS）理念的宣传教育和相应的呼吸道综合管理；对照组仅进行日常的手术前宣传教育和护理知识指导。对两组患者肺功能、手术后引流量、拔管所需时间、手术后住院时长以及住院费用进行对比。

结果

试验组患者术后拔管时间、术后住院时间、住院费用以及并发症发生率均低于对照组，并且肺癌患者手术后肺功能恢复状况优于对照组。

结论

遵循ERAS理念并接受呼吸道综合管理的肺癌患者，手术后并发症的发生率明显降低，住院时间明显缩短，肺功能能够得到较大程度的恢复，从而缓解住院给家庭带来的经济压力，节省医疗资源。

Abstract: Objective

To analyze and study the effect of comprehensive respiratory management under the concept of rapid recovery during perioperative period on the rapid recovery of lung cancer patients.

Methods

Lung cancer patients admitted to the Department of Thoracic Surgery of the Second Affiliated Hospital of Shandong First Medical University from August 2020 to August 2021 were collected. The experiment was divided into treatment group （patients who underwent integrated respiratory management） and control group （patients who received routine care） according to the digital odd-even method. For the respiratory function exercise group， a series of ERAS' concept education and corresponding integrated respiratory management were received before surgery. The control group had only daily pre-operation education and nursing knowledge guidance. Lung function， postoperative drainage，extubation time， postoperative length of hospital stay and hospitalization expenses were compared between the two groups.

Results

Patients in the comprehensive respiratory function exercise group' postoperative extubation time， postoperative hospitalization time， cost of hospitalization and complication rate were lower than those of the control group， and lung cancer patients recovered better than those in the control group.

Conclusion

The patients with lung cancer who follow the ERAS concept and receive integrated respiratory management can recover their lung function to a greater extent， the incidence of postoperative complications in patients with lung cancer can be reduced， the length of hospital observation is shortened， thus relieving the economic pressure brought by hospitalization on their families， and effectively utilizing hospital beds.
