摘要/Abstract

摘要：

目的分析他汀类药物使用情况，围绕不合理处方提出合理用药建议，促进他汀类药物安全、有效、经济、适宜使用。方法从青岛大学附属泰安市中心医院SPSS系统调取2020年全年门诊以及住院患者使用的所有降血脂类药物的规格、剂型、药物消耗总量、消耗金额、通用名、用药频度（defined daily doses，DDDs）等信息，统计分析各类降血脂药物使用情况以及他汀类药物地位，重点分析他汀类药物使用情况，并回顾性抽取、点评此时期内使用他汀类药物的门诊处方，在分析不合理处方基础上，给出合理用药建议。结果该院所用降血脂药物中，他汀类药物使用频率最高，DDDs构成比37.3%，他汀类药物中瑞舒伐他汀使用率最高（47.68%）；随机抽取1 200张使用他汀类药物的处方中，有113张不合理处方（不合理处方率9.42%），主要涉及适用证不适宜、用法用量不当、配伍不符以及处方不规范等问题，其中，适应证不适宜占所有不合理处方比例最高（55.10%）；针对具体用药不合理问题，提出定期抽查处方等4条建议。结论该院他汀类药物使用频率较高，存在不合理使用情况，需在具体工作中结合具体患者疾病特点不断完善合理用药方案，加强临床药学服务，提高用药合理性。

Abstract:

ObjectiveTo analyse and review the use of statins in a hospital and make recommendations for rational use around unreasonable prescriptions to promote the safe， effective， economical and appropriate use of statins.MethodsFrom the SPSS system of our hospital， we retrieved the specifications， dosage forms， total drug consumption， consumption amount， generic names， DDDs and other relevant information of all lipid-lowering drugs in outpatient as well as inpatient for the whole year of 2020， statistically analysed the use of various types of lipid-lowering drugs and the status of statins with further focus on statin use， retrospectively extracted outpatient prescriptions using statins during the review period， and gave rational drug use recommendations based on analysis of irrational prescriptions.ResultsAmong the lipid-lowering drugs used in the hospital， statins were the most frequently used， with a DDDs composition ratio of 37.3%， and among the statins， Rosuvastatin had the highest rate of use （47.68%）. Among them， inappropriate indications accounted for the highest proportion of all irrational prescriptions （55.10%）； four recommendations were made to address specific irrational drug use problems， including regular sample checks of prescriptions.ConclusionThe hospital has a high frequency of statin use， but there are still cases of irrational use. After the clinical pharmacist has made recommendations for rational use of drugs， it is still necessary to continuously improve the rational use of drugs specifically in conjunction with the characteristics of specific patients' diseases， strengthen pharmacy services and improve the rationality of clinical drug use.
