血清MK及bFGF联合测定对胃癌术后复发的预测价值

Analysis on predictive value of combined detection of serum MK and bFGF in predicting postoperative recurrence of patients with early gastric cancer

摘要: 目的

探究血清中期因子(midkine,MK)和碱性成纤维细胞生长因子(basic fibroblast growth factor,bFGF)联合测定对胃癌术后复发的预测价值。

方法

收集2015年1月至2019年12月我院确诊并收治的165例原发性胃癌患者作为模型组,另纳入2020年1月至2020年5月我院确诊并收治的45例原发性胃癌患者作为验证组,根据根治性手术治疗后是否复发分为复发组和未复发组,采用酶联免疫吸附法(ELISA)检测所有患者血清MK和bFGF的表达水平,并采用受试者工作特征曲线(receiver operating characteristic curve,ROC)判定各指标单独检测和联合检测的预测价值,并验证预测效能。

结果

模型组中复发患者血清MK和bFGF的表达水平显著高于未复发患者(P< 0.05)。MK检测的临界值为232.41 pg/mL,对应的敏感度为50.00%,特异度为69.84%,ROC曲线下面积(AUC)为0.731(95%CI:0.657 ~ 0.779);bFGF检测的临界值为404.24 pg/mL,对应的敏感度为62.50%,特异度为71.43%,AUC为0.725(95%CI:0.629 ~ 0.768);MK + bFGF并列联合检测对胃癌复发预测的敏感度为87.50%,特异度为74.60%,AUC为0.889(95%CI:0.831 ~ 0.924),高于MK和bFGF单独检测的预测价值。验证组AUC为0.805(95% CI:0.759 ~ 0.839),预测效能较高。

结论

血清MK和bFGF表达水平可作为胃癌根治术后复发的重要预测指标,且两者联合检测具有更高的预测价值。

Abstract: Objective

To investigate the predictive value of combined detection of serum midkine (MK) and basic fibroblast growth factor (bFGF) in predicting postoperative recurrence of patients with early gastric cancer.

Methods

165 patients with early gastric cancer admitted to our hospital from January 2015 to December 2019 were collected as model group and divided into recurrence subgroup and non-recurrence subgroup, while 45 patients with early gastric cancer admitted to our hospital from January 2020 to May 2020 were collected as verification group. The serum MK and bFGF of all patients were detected by ELISA, the predictive value of serum MK and bFGF were evaluated by receiver operating characteristic curve(ROC), and the prediction efficiency was verified.

Results

The expression levels of serum MK and bFGF in recurrence subgroup of the model group were higher as compared with non-recurrence subgroup (P< 0.05). ROC analysis showed that the critical value, sensitivity, specificity and AUC of MK were 232.41 pg/mL, 50.00%, 69.84% and 0.731 (95%CI: 0.657~0.779), the critical value, sensitivity, specificity and AUC of bFGF were 404.24 pg/mL, 62.50%, 71.43% and 0.725(95%CI: 0.629~0.768), and the sensitivity, specificity and AUC of MK + bFGF were 87.50%, 74.60% and 0.889(95%CI: 0.831~0.924), which were higher than the predictive value of MK or bFGF. The AUC of verification group was 0.805 (95%CI: 0.759~0.839), which was a high prediction efficiency.

Conclusion

The expression levels of serum MK and bFGF can be important factors in predicting recurrence of patients with early gastric cancer, and combined detection of MK and bFGF has higher predictive value.
