宫腔粘连术后妊娠结局的临床分析

摘要： 目的

研究169例宫腔粘连（intrauterine adhesions，IUA）患者的临床资料及病例特征，分析宫腔镜治疗后患者的月经情况、复发因素及妊娠结局等。

方法

回顾性分析经宫腔镜下宫腔粘连分离术（trans cervical resection of adhesions，TCRA）治疗的169例宫腔粘连患者的临床资料，随访患者术后月经及妊娠等情况，采用卡方检验和单因素分析，评估可能影响术后妊娠的因素。

结果

169例患者IUA治疗后月经改善且未复发的共121例，治疗有效率为71.60%。169例患者术后有66例妊娠，术后妊娠率为39.05%，其中轻度IUA妊娠率为56.25%，中重度IUA妊娠率为37.91%。

结论

冷刀手术方式、术后运用物理屏障和人工周期药物可有效改善月经或预防IUA复发；分离粘连的手术方式是影响术后妊娠率的重要因素。

Abstract: Objective

To analyze the menstrual conditions， recurrence factors and pregnancy outcomes of patients after hysteroscopic treatment.

Methods

A retrospective study was conducted on the clinical data of 169 patients with transcervical resection of adhesions （TCRA）. The postoperative menstruation and pregnancy of these patients were followed-up. Chi-squared test and One-Way ANOVA analysis were used to evaluate the factors that might affect postoperative pregnancy.

Results

121 patients had improved menstruation and didn't recur （71.60%）. 66 of the 169 patients had pregnancies （39.05%）. Among them， the pregnancy rate of mild IUA was 56.25%， and the rate from the moderate to severe IUA was 37.91%.

Conclusion

Cold knife surgery， postoperative use of physical barrier and artificial cycle drugs can effectively improve menstruation or prevent IUA recurrence. The surgical method of isolating adhesions is an important factor affecting the pregnancy rate after surgery.
