目的探讨胆固醇水平与2型糖尿病中青年男性骨代谢的相关性。方法选择79例2型糖尿病中青年男性,根据总胆固醇(total cholesterol,TC)水平分为正常TC组(TC < 4.5 mmol/L)及高TC组(TC ≥ 4.5 mmol/l/L),检测所有患者的血脂[TC、甘油三脂(triglyceride, TG)、高密度脂蛋白胆固醇(high density lipoprotein-cholesterol, HDL-C)、低密度脂蛋白胆固醇(low density lipoprotein-cholesterol, LDL-C)]、糖化血红蛋白(HbA1c)及骨代谢标志物[甲状旁腺素(parathyroid hormone, PTH)、25羟维生素D(25-OH-D)、骨钙素(osteocalcin, OC)、血清Ⅰ型前胶原N-末端前肽(P1NP)、Ⅰ型胶原交联C-末端肽(β-CTX)]和骨密度(bone mineral density, BMD),分析血脂与骨密度及骨代谢标志物间的相关性。结果校正年龄、糖化血红蛋白、BMI的影响后, 2型糖尿病中青年男性患者胆固醇水平与腰椎(L1 ~ 4)BMD呈负相关(r= -0.266,P= 0.034),LDL-C水平与PTH呈正相关(r= 0.244,P= 0.042)。结论高胆固醇血症与2型糖尿病中青年男性腰椎骨密度降低有关且独立于年龄、 BMI及血糖控制情况。
