2型糖尿病中青年男性胆固醇水平与骨代谢的相关性

摘要:

目的探讨胆固醇水平与2型糖尿病中青年男性骨代谢的相关性。方法选择79例2型糖尿病中青年男性,根据总胆固醇(total cholesterol,TC)水平分为正常TC组(TC < 4.5 mmol/L)及高TC组(TC ≥ 4.5 mmol/l/L),检测所有患者的血脂[TC、甘油三脂(triglyceride, TG)、高密度脂蛋白胆固醇(high density lipoprotein-cholesterol, HDL-C)、低密度脂蛋白胆固醇(low density lipoprotein-cholesterol, LDL-C)]、糖化血红蛋白(HbA1c)及骨代谢标志物[甲状旁腺素(parathyroid hormone, PTH)、25羟维生素D(25-OH-D)、骨钙素(osteocalcin, OC)、血清Ⅰ型前胶原N-末端前肽(P1NP)、Ⅰ型胶原交联C-末端肽(β-CTX)]和骨密度(bone mineral density, BMD),分析血脂与骨密度及骨代谢标志物间的相关性。结果校正年龄、糖化血红蛋白、BMI的影响后, 2型糖尿病中青年男性患者胆固醇水平与腰椎(L1 ~ 4)BMD呈负相关(r= -0.266,P= 0.034),LDL-C水平与PTH呈正相关(r= 0.244,P= 0.042)。结论高胆固醇血症与2型糖尿病中青年男性腰椎骨密度降低有关且独立于年龄、 BMI及血糖控制情况。

Abstract:

ObjectiveTo investigate the correlation between total cholesterol level and bone metabolism in young and middle-aged patients with type 2 diabetes.MethodsSeventy-nine young and middle-aged patients with type 2 diabetes were divided into two groups,according to TC level: high cholesterol group (TC ≥ 4.5 mmol/l/L) and normal cholesterol level group (TC < 4.5 mmol/L) The serum lipids [total cholesterol (TC), triglyceride (TG), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C)], glycated hemoglobin and bone mineral density (BMD), and bone metabolism markers were detected and correlation between blood lipid, bone mineral density and bone metabolic markers was analyzed.ResultsAfter the adjusting of age, HbA1c and BMI, cholesterol level was negatively correlated with lumbar bone mineral density in young and middle-aged men with Type 2 diabetes(r= -0.266,P= 0.034) and LDL-C level is positively correlated with PTH (r= 0.244,P= 0.042).ConclusionThe high cholesterol level is associated with decreased lumbar bone density in young and middle-aged patients with Type 2 diabetes and is independent of age, BMI, and glycemic control.
