Meta-analysis of different medical imaging techniques applied in the diagnosis of anal fistula

摘要:

目的对3种不同的影像诊断方法:经直肠三维超声(three-dimensional endoanal ultrasound,3D-EAUS)、多层计算机断层扫描(multi-slice computed tomography, MSCT)及磁共振成像(magnetic resonance imaging,MRI)在肛瘘诊断中的应用进行系统评价和Meta分析,为推荐较优的影像学诊断方法提供可靠证据。方法采用RevMan 5.4.1软件进行诊断性试验的Meta分析,比较3D-EAUS、MRI、MSCT在肛瘘诊断应用中的灵敏度(sensitivity,SEN)、特异度(specificity,SPE)、集成受试者工作特征(summary receiver operating characteristic,SROC)曲线主要结局指标的差异。结果3D-EAUS、MSCT及MRI的SROC曲线显示大多数点落在曲线上或位于曲线的左上角,提示3者均有较好的诊断效果。3D-EAUS对肛管括约肌间型和经肛管括约肌型的诊断灵敏度较高,但对肛管括约肌上型和肛管括约肌外型的灵敏度相对较低;MSCT对区分高位复杂瘘、低位复杂瘘管及内口数量的灵敏度较低;MRI无论是平扫还是多序列联合下对肛瘘的诊断,其灵敏度、特异度均优于3D-EAUS和MSCT。结论MRI可作为诊断肛瘘较优的影像学选择。

Abstract:

ObjectiveThe application of three different diagnostic methods (EAUS, MSCT, MRI) in anal fistula diagnosis was systematically evaluated. Meta analysis was conducted to provide reliable evidence for recommending better diagnostic methods.MethodsMeta-analysis of diagnostic tests was conducted by RevMan 5.4.1 software, parks classification,high and low complex fitula classification of anal fistula was used as the grouping basis to mainly compare the differences of major outcome indicators such as SEN, SPE, PLR, NLR and SROC curves applied by three different imaging diagnostic methods.ResultsThe sensitivity of EAUS were higher for interanal sphincter and transanal sphincter, but lower for supra-anal sphincter and external anal sphincter. The sensitivity and specificity of CT were lower for the number of high and low complex fistulas. In contrast, whether MRI is plain scan or multi-sequence combination, its sensitivity and specificity are superior to EAUS and MSCT in the diagnosis of anal fistula, suggesting that MRI can be a better choice for the diagnosis of anal fistula.ConclusionMRI is a recommended auxiliary examination method with clinical significance and value.
